目的探讨孟鲁司特对呼吸道合胞病毒(RSV)毛细支气管炎患儿血清半胱氨酰白三烯(Cys-LTs)的影响。方法将68例2~18个月的RSV毛细支气管炎患儿随机分为观察组和常规组,每组34例。常规组予综合治疗,观察组在此基础上联合应用孟鲁司特4 mg,每晚1次口服。于治疗前后检测血清Cys-LTs水平,并观察2组患儿疗效及症状、体征恢复正常时间;健康对照组32例为门诊体检健康者,男17例,女15例。结果治疗前观察组和常规组RSV毛细支气管炎Cys-LTs水平均明显高于健康对照组(P<0.01);治疗后观察组症状和体征恢复正常时间均较常规组早(Pa<0.01),Cys-LTs水平较常规组显著下降(P<0.05),常规组血清Cys-LTs水平仍高于健康对照组(P<0.05)。结论孟鲁司特可降低RSV毛细支气管炎患儿血清Cys-LTs水平,在RSV毛细支气管炎的抗炎机制中发挥重要作用,并可缩短病程。
Objective To investigate the effect of montelukast on serum cysteinyl leukotrienes (Cys-LTs) in children with respiratory syncytial virus (RSV) bronchiolitis. Methods 68 children aged 2-18 months with RSV bronchiolitis were randomly divided into observation group and conventional group, 34 cases in each group. The conventional group was given comprehensive treatment. The observation group was given montelukast 4 mg orally on the basis of this once a day. The serum Cys-LTs levels were measured before and after treatment. The curative effect, symptoms and signs of normal children in two groups were observed. In the healthy control group, 32 cases were out-patient physical examination, 17 males and 15 females. Results The levels of Cys-LTs in the RSV bronchiolitis group were significantly higher than those in the healthy control group (P <0.01) before treatment and those in the control group were earlier (P <0.01) than those in the control group The level of Cys-LTs was significantly lower than that of the control group (P <0.05), while the level of Cys-LTs in the routine group was still higher than that of the healthy control group (P <0.05). Conclusion Montelukast can reduce the level of serum Cys-LTs in children with RSV bronchiolitis, play an important role in the anti-inflammatory mechanism of RSV bronchiolitis, and can shorten the course of disease.
