目的探讨Elrod椎间动态稳定系统在治疗腰椎不稳症的临床应用和疗效。方法回顾性分析2012年2月~2013年3月使用腰椎后路Elrod椎间动态稳定系统固定治疗腰椎不稳症20例患者资料，男11例，女9例；年龄23~59岁，平均44.9岁。患者均伴有不同程度的活动性腰背疼痛症状，静息位时缓解。采用视觉模拟评分(visual analogue scale，VAS)对手术前后腰背、下肢疼痛进行评估；采用Oswestry功能障碍指数(Oswestry disability index，ODI)对手术前后腰椎功能进行评价。测量术前及术后手术节段椎间隙前后缘高度、前凸角以及活动度，并进行比较。结果20例患者均获得随访，随访时间12~24个月，平均(18±2.9)个月。术后4w及末次随访腰背部VAS评分为(2.1±0.6)分和(1.3±0.5)分，与术前(7.6±2.2)分比较，差异有统计学意义；术后4w及末次随访ODI为52.4豫±13.8豫和10.3豫±2.3豫，与术前81.1豫±12.1豫比较差异有统计学意义。术后4w手术节段前凸角及节段活动度分别为7.62o±0.68o及4.34o±0.79o。患者切口均一期愈合，无1例发生脊髓损伤、脑脊液漏及内固定装置移位、脱出等并发症，治疗效果满意。结论 Elrod椎间动态稳定系统可预防和减少手术节段的椎间盘退变，维持固定节段的椎间高度与活动度，近期效果满意，中远期效果有待于进一步临床观察。“,”Objective To investigate the clinical application and ef icacy of Elrod Intervertebral Dynamic Stabilization System for treatment of lumbar instability. Methods Data of 20 patients who had implant Elrod Intervertebral Dynamic Stabilization System for lumbar instability from February 2012 to April 2013 were retrospectively analyzed.There were 11 males and 9 female,aged from 23 to 59 years (average 44.9 years).Al patients suf ered from varying degrees of lower back pain induced by exercise and relieved by rest.The visual analogue scale (VAS) was used to evaluate lower back and limbs pain. Oswestry disability index (ODI) was used to evaluate lumbar function.The pre-and postoperative anterior and posterior disc height, segmental lordotic angle and lumbar range of motion at operative level were tested and compared.Results Al patients were fol owed up for 12 to 24 months.The VAS score improved from preoperative 7.6±2.2 to 2.1±0.6 at 4 weeks postoperatively and 1.3±0.5 at final fol ow-up,the dif erences were statistical y significant ( <0.05).The ODI improved from preoperative 81.1%±12.1% to 52.4%±13.8% at 4 weeks postoperatively and 10.3%±2.% at final fol ow-up,the dif erences were statistical y significant( <0.05),. The postoperative segmental lordotic angle and lumbar range of motion at 4 weeks were 7.62o±0.68oand 6.34o±0.79o,respectively.Wound healed smoothly in al patients,and there were no complications such as spinal cord injury,cerebrospinal fluid leakage,device displacement and device dislocation.Conclusion Elrod Intervertebral Dynamic Stabilization System can prevent and decrease the potential risk of operative and adjacent level degeneration as wel as maintain the disc height and motion range of operative level,the short and mid-term ef icacy is satisfactory,but we need further observation to assess the long-term ef icacy.
