目的对深圳市近期发生的一起不明原因群体性腹泻进行现场流行病学调查和实验室检测,探讨该次群体性腹泻的原因,提出应对类似爆发流行的防制策略。方法描述性流行病学分析爆发过程的三间分布,对采集的18份肛拭子以细菌学培养、分子生物学分析和脉冲场凝胶电泳分型(PFGE)分析等方法进行实验室检测。结果18份肛拭子中检出4株肠产毒性大肠杆菌,证实为同一血清型O78:K80,菌株同源性达到96.3%。结论本次在居民小区发生的爆发的群体性腹泻致病菌为产毒性大肠杆菌(ETEC),小区二次供水系统的渗露和连续的暴雨可能是污染的来源,居民个人卫生习惯不良是导致爆发的重要因素。建议对二次供水系统开展季节性检修,并对居民实施有针对性的健康教育,将有利于预防此类疫情的发生。
Objective To investigate the epidemiological and laboratory tests of an unknown group of diarrhea of unknown origin in Shenzhen in recent years and explore the causes of this group of diarrhea and propose a strategy to deal with the epidemic of similar outbreaks. Methods Descriptive epidemiology was used to analyze the three distributions of the outbreaks. The laboratory tests were performed on 18 collected samples of rectal swabs by bacteriological culture, molecular biological analysis and PFGE analysis. Results Four strains of enterotoxigenic Escherichia coli were detected in 18 rectal swabs. The homology was 96.3% for the same serotype O78: K80. Conclusions This outbreak of community-based diarrhea-causing pathogenic bacteria in residential communities is endotoxin-producing Escherichia coli (ETEC). The infiltration of secondary water supply system and continuous rainstorm may be sources of pollution. Residents’ personal hygiene habits are poor An important factor in the outbreak. It is suggested that carrying out seasonal maintenance on the secondary water supply system and implementing targeted health education on residents will help prevent such outbreaks.
