目的:探索妊娠滋养细胞肿瘤与子宫血管的关系。方法:甲基丙烯酸酯为铸型剂,对离体10例子宫(绒癌3例、葡萄胎2例、侵蚀性葡萄胎3例、对照2例)进行灌注,制成标本,扫描电镜观察血管的异常改变。结果:正常子宫血管分为二种类型,一是树枝状分布,二是螺旋状血管;良性葡萄胎的血管改变无明显特征;恶性滋养细胞肿瘤血管铸型立体构式,大体仍是树枝分布,螺旋血管增多,可见大片的血窦状改变血管、发夹状血管、串珠状改变及多处铸型剂漏出等血管被破坏性改变,但最为明显的改变是新生血管的出现;侵蚀性葡萄胎与绒癌的血管异常改变无明显的差异。结论:妊娠滋养细胞肿瘤在破坏子宫血管的同时,伴有新生血管的形成,为肿瘤的抗血管形成治疗提供有力证据。
Objective: To explore the relationship between gestational trophoblastic tumor and uterine blood vessels. Methods: Methacrylate was used as a mold preparation and 10 cases of uterus (3 cases of choriocarcinoma, 2 cases of hydatidiform mole, 3 cases of invasive hydatidiform mole and 2 cases of control) were perfused with 10% Abnormal changes. Results: Normal uterine blood vessels were divided into two types. One was dendritic distribution, the other was spiral blood vessels. No significant changes were found in the vascular changes of benign hydatidiform mole. The three-dimensional configuration of malignant trophoblastic tumor blood vessels was still mostly branch distribution, Spiral blood vessels increased, showing large blood sinusoidal changes in blood vessels, hairpin vessels, beaded changes and multiple mold leaks and other blood vessels were destructive changes, but the most obvious change is the emergence of new blood vessels; invasive mole Vascular abnormalities and choriocarcinoma no significant difference. Conclusion: Gestational gestational trophoblastic tumor destroy the uterine vessels at the same time, accompanied by the formation of new blood vessels, the anti-angiogenesis of the tumor provides strong evidence of treatment.
