目的:观察板层蛋白(laminin,LN)在儿童系膜增生性肾小球肾炎(MsPGN)肾活检肾组织中表达的质或量的改变及其意义。方法:以板层蛋白α_1、α_2、β_1、β_2、γ_1结构链为观察对象,用免疫组化和原位杂交技术观察它们在18例系膜增生性肾小球肾炎肾活检组织和6例正常成熟肾组织中的表达和分布。结果:①LNα_1;、LNγ_1在肾小球系膜增殖节段蛋白表达增强;LNα_2、LNβ_1在部分增殖的肾小球系膜区出现节段性异常表达;LNβ_2链在增生的系膜区和肾小球基膜(GBM)表达出现不同程度的下降。②LNα_1、α_2、β_1、和γ_1的mRNA在系膜增生的肾小球内表达有不同程度的增加,但轻度、中度和重度系膜增生的组间无明显差异。结论:①在系膜增生性肾小球肾炎中板层蛋白的表达既有量的改变也有质的变化,增生的系膜细胞是肾小球内板层蛋白量的积聚和异常合成的重要细胞来源。②板层蛋白各分子链分布的改变可能是肾小球功能异常的病理基础。
OBJECTIVE: To investigate the changes of the expression of laminin (LN) in renal tissues of children with mesangial proliferative glomerulonephritis (MsPGN) and its significance. Methods: The structural strands of laminin α_1, α_2, β_1, β_2 and γ_1 were used as the observation objects. Immunohistochemistry and in situ hybridization were used to observe their expression in 18 cases of mesangial proliferative glomerulonephritis with renal biopsies and 6 cases of normal Mature renal tissue expression and distribution. Results: ①The expression of LNα_1, LNγ_1 in glomerular mesangial proliferative segments increased; LNα_2, LNβ_1 were abnormally expressed in partially proliferated glomerular mesangial regions; LNβ_2 chain was found in proliferative mesangial and renal tissues Globular basement membrane (GBM) expression decreased to varying degrees. ② The mRNA expressions of LNα_1, α_2, β_1, and γ_1 increased to some extent in mesangial proliferative glomeruli, but there was no significant difference between mild, moderate and severe mesangial proliferative groups. Conclusions: ① The expression of laminin in mesangial proliferative glomerulonephritis is both changeable and qualitative, and the mesangial cells are the important cells for accumulation and abnormal synthesis of glomerular platelet protein source. ② The distribution of lamellar protein molecular chains may be the pathological basis of glomerular dysfunction.
