白内障囊外摘出术中,12点钟处的晶体皮质最难吸出,在瞳孔变小时尤其如此。而上方皮质会附在人工晶体的前面或后面引起暂时或永久的视力障碍。在非超声乳化吸出时,切口可作得稍大使虹膜能够嵌入以获得上方晶体赤道的极好暴露。虹膜切除法也可使上方皮质清除干净,但会影响视力的增进,故作者推荐虹膜嵌顿法。此法用于75例病人,有1例发生虹膜囊肿,激光治疗后消失。还不能肯定囊肿是因嵌顿虹膜引起的。
In extracapsular cataract extraction, the crystalline cortex at 12 o’clock is most difficult to aspirate, especially when the pupil becomes smaller. The upper cortex will be attached to the front or back of the intraocular lens causing temporary or permanent visual impairment. In non-phacoemulsification aspiration, the incision can be made slightly larger so that the iris can embed in order to obtain an excellent exposure of the upper crystalline equator. Iris excision can also make the top of the cortex clean, but will affect the vision improvement, so the author recommends iris incarceration. This method is used in 75 patients, one case of iris cyst, disappeared after laser treatment. Still not sure cyst is caused by incarcerated iris.
