目的探讨周围型肺癌螺旋CT(spiralCT,SCT)表现特征与癌组织中碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)的表达及微血管生成之间的关系。方法对22例经手术病理证实且行SCT平扫和增强扫描的周围型肺癌病灶进行回顾性分析,用免疫组化SP法检测肿瘤组织bFGF的表达以及微血管密度(microvessel density,MVD),将周围型肺癌的SCT表现特征分别与免疫组化结果进行对照分析。结果bFGF在周围型肺癌组织中的阳性表达率为59.9%,平均MVD为46.10±18.18,MVD在bFGF表达的阳性、阴性组具有显著差异(P<0.05)。周围型肺癌SCT图像上瘤体大小、纵隔淋巴结增大及深分叶征与MVD有相关性(P<0.05);棘突征、血管集束征及肿瘤的强化程度与bFGF及MVD均有相关性(P<0.05);毛刺征、空洞征、支气管气像及胸膜凹陷征与bFGF及MVD均无明显相关性(P>0.05)。结论SCT的表现特征在一定程度上可反映周围型肺癌的微血管生成以及bFGF的表达情况。
Objective To investigate the relationship between the expression of basic fibroblast growth factor (bFGF) and the expression of angiogenesis in peripheral lung cancer. Methods Twenty-two cases of peripheral lung cancer confirmed by pathology and SCT scan and enhanced scan were retrospectively analyzed. The expression of bFGF and microvessel density (MVD) in tumor tissue were detected by immunohistochemical SP method. SCT characteristics of lung cancer and immunohistochemistry were analyzed. Results The positive expression rate of bFGF in peripheral lung cancer tissues was 59.9% and the average MVD was 46.10 ± 18.18. There was a significant difference in the expression of bFGF between bFGF and negative group (P <0.05). The size of tumor, the enlargement of mediastinal lymph nodes and the relationship between deep lobulation and MVD in SCT images of peripheral lung cancer were correlated with MVD (P <0.05). The degree of enhancement of spinous process, tumor clustering and tumor were correlated with bFGF and MVD (P <0.05). There were no significant correlations between bFGF and MVD (P> 0.05) in spiculation, cavitation, bronchial airway and pleural indentation. Conclusions The characteristics of SCT may reflect the angiogenesis and the expression of bFGF in peripheral lung cancer to a certain extent.
