环孢素(Cyclosporine,Cy)是一种有效的免疫抑制剂,广泛用于器官移植。然而Cy有许多令人讨厌的副作用,包括肾毒性,以及近来报告的可能对动物和人胰岛β细胞的毒性。Cy可导致葡萄糖耐量损害、胰岛素分泌下降及周围胰岛素抵抗增加。由于人体试验中,大多数病人除用Cy外,还应用糖皮质激素,众所周知后者是致糖尿病药物,所以本文作者研究了单独应有Cy对肾移植病人的致糖尿病作用。因肾移植入院的非糖尿病人禁食过夜后进行研究。免疫抑制仅用Cy,开始10mg/kg,然后调整到维持全血Cy水平在200~500mg/ml之间(非特异放射免疫测定法)。排异发作给予甲基强的松龙0.5g
Cyclosporine (Cy) is a potent immunosuppressive agent widely used in organ transplantation. However, Cy has a number of nasty side effects including nephrotoxicity and recently reported potential toxicity to animal and human pancreatic β-cells. Cy can lead to impaired glucose tolerance, decreased insulin secretion, and increased peripheral insulin resistance. Because most patients used glucocorticoids in addition to Cy, the latter is known to cause diabetes mellitus, so the authors studied the diabetes-causing effects of Cy on renal transplant alone. Non-diabetic patients admitted to hospital for kidney transplantation were fasted overnight.
