:目的 探讨特异性基因标志对急性淋巴细胞白血病(ALL)患者中枢神经系统白细胞(CNSL)早期诊断的价值。方法 用聚合酶链反应(PCR)方法扩增ALL患者不同期脑脊液免疫球蛋白重链(IgH)基因重排和T细胞受体γ(TCRγ)基因重排。结果 12例ALL脑脊液标本两种PCR基因扩增6例(9份)获阳性结果,与临床结果相符;2例患者临床不符CNSL,但PCR阳性,经观察治疗后确诊。结论 本法特异性强,且有较高的灵敏度,可作为ALL患者CNSL早期诊断的有效方法之一。
Objective To investigate the value of specific gene markers in the early diagnosis of central nervous system leukocyte (CNSL) in patients with acute lymphoblastic leukemia (ALL). Methods The levels of immunoglobulin heavy chain (IgH) gene rearrangements and T cell receptor γ (TCRγ) gene rearrangements in patients with ALL were amplified by polymerase chain reaction (PCR). Results The positive results were obtained in 6 cases (9 samples) from 12 cases of cerebrospinal fluid (CSF) samples amplified by PCR, which accorded with the clinical results. The 2 patients were clinically incompatible with CNSL, but the PCR was positive and confirmed by observation and treatment. Conclusion This method is of high specificity and high sensitivity and can be used as an effective method for the early diagnosis of CNSL in ALL patients.
