[目的]比较3种疗法治疗2型糖尿病患者初发性翼状胬肉的临床疗效分析。[方法]糖尿病翼状胬肉112例(118眼),实行随机双盲,分别采用0.1%丝裂霉素C+单纯切除术(A组,36眼)、0.1%丝裂霉素C+羊膜移植术(B组,42例)、0.1%丝裂霉素C+自体带球结膜瓣的角膜缘干细胞移植术(C组,40眼),术后跟踪随访9～18个月,比较创面修复时间及胬肉复发率。[结果]复发率A组33.3%(12/36),B组9.5%(4/42),C组7.5%(3/40),C组较低(2=11.449,P<0.05);角膜上皮愈合时间A组(3.49±0.66)d,B组(7.83±0.79)d,C组(5.65±0.71)d,A组愈合较快。[结论]采用0.1%丝裂霉素C+自体带球结膜瓣的角膜缘干细胞移植术治疗糖尿病患者初发翼状胬肉,既有效预防复发,又减少并发症。
[Objective] To compare the clinical efficacy of three kinds of therapy in the treatment of type 2 diabetes patients with primary pterygium. [Methods] One hundred and twelve patients (118 eyes) with pterygium were randomly divided into two groups. The patients were treated with 0.1% mitomycin C + resection alone (group A, 36 eyes) and 0.1% mitomycin C + amniotic membrane transplantation (Group B, n = 42) and limbal stem cell transplantation (group C, 40 eyes) with 0.1% mitomycin C + autotrophic conjunctival flap. The patients were followed up for 9-18 months. The wound healing time and pterygium Recurrence rate. [Results] The recurrence rate was 33.3% (12/36) in group A, 9.5% (4/42) in group B, 7.5% (3/40) in group C and lower in group C (2 = 11.449, P <0.05) The healing time of epithelial healing in group A was (3.49 ± 0.66) d, in group B (7.83 ± 0.79) d, in group C (5.65 ± 0.71) d, and in group A was faster. [Conclusion] The limbal stem cell transplantation with 0.1% mitomycin C + autogenous conjunctival flap for the treatment of primary pterygium in patients with diabetes is effective in preventing recurrence and reducing complications.
