目的:检测慢性乙型肝炎孕妇新生儿脐带血T淋巴细胞亚群、HBV标志物和孕妇肝功能的变化,探讨孕妇乙型肝炎病毒(HBV)感染对新生儿脐血T淋巴细胞亚群的影响。方法:选择38例HBV携带者产妇为观察组,15例HBsAg阴性健康产妇作为对照组,收集产妇静脉血和脐带血。检测脐血T淋巴细胞亚群(包括CD3+,CD4+,CD8+,CD19+,CD16+CD56+,CD4+/CD8+)的百分比、孕妇HBV标志物、脐血的HBV标志物。分析比较脐血不同HBV标志物模型与T淋巴细胞亚群的关系及T淋巴细胞亚群与肝功能的相关性。结果:①感染组和对照组脐血HBV标志物各模式对其脐血T淋巴细胞亚群无明显影响,差异无统计学意义(P>0.05)。②感染组和对照组肝功能对脐血的细胞免疫无明显影响,差异无统计学意义(P>0.05)。③HBsAg阴性健康产妇组与HBeAg阳性HBsAg携带者、HBeAg阴性HBsAg携带者分娩的新生儿细胞免疫状况比较,差异均无统计学意义(P>0.05)。结论:①HBV孕妇所分娩的新生儿脐血T淋巴细胞亚群无比例失衡。②脐血T淋巴细胞之间有内在的协助与互相制约的动态平衡关系。
Objective: To detect the changes of neonatal umbilical cord blood T lymphocyte subsets, HBV markers and pregnant women ’s liver function in chronic hepatitis B and explore the influence of hepatitis B virus (HBV) infection on the T lymphocyte subsets of neonatal cord blood . Methods: 38 cases of HBV carriers were selected as the observation group and 15 cases of HBsAg-negative healthy women as the control group. Maternal venous blood and umbilical cord blood were collected. The percentage of T lymphocyte subsets (including CD3 +, CD4 +, CD8 +, CD19 +, CD16 + CD56 +, CD4 + / CD8 +) in cord blood, HBV markers in pregnant women and HBV markers in cord blood were detected. To analyze the relationship between different HBV markers in cord blood and T lymphocyte subsets and the relationship between T lymphocyte subsets and liver function. Results: ① The patterns of cord blood HBV markers in infected and control groups had no significant effect on T lymphocyte subsets in cord blood, with no significant difference (P> 0.05). ②Infected group and control group, liver function had no significant effect on cord blood cellular immunity, the difference was not statistically significant (P> 0.05). (3) There was no significant difference in cellular immune status among newborns with HBsAg-negative healthy mothers group and HBeAg-positive HBsAg carriers and HBeAg-negative HBsAg carriers (P> 0.05). Conclusion: (1) UB-T lymphocyte subpopulations of neonates delivered by HBV pregnant women have a disproportionate imbalance. ② umbilical cord blood T lymphocytes between the intrinsic assistance and mutual restraint of the dynamic equilibrium relationship.
