比较七氟醚、安氟醚对颅内压(ICP)和脑灌注压(CPP)的影响,评估了七氟醚在颅脑外科的临床特征.方法:观察33例择期经蝶垂体腺瘤切除术患者,随机分为2组,七氟醚组16例,安氟醚组17例.吸入七氟醚或安氟醚,调整其浓度依次为0.5,1.0,1.5最小肺泡浓度(MAC),监测ICP,CPP,平均动脉压(MAP)及心率(HR).结果:七氟醚、安氟醚均可引起ICP增高,CPP下降,并使MAP,HR下降,且都与吸入浓度呈正相关,安氟醚改变程度较七氟醚为甚.结论:七氟醚、安氟醚所致ICP,CPP及MAP改变均在正常范围之内,且七氟醚对上述变化幅度均较安氟醚小.提示七氟醚更适宜在颅脑外科麻醉中使用.
The effects of sevoflurane and enflurane on intracranial pressure (ICP) and cerebral perfusion pressure (CPP) were compared and the clinical characteristics of sevoflurane in craniocerebral surgery were evaluated. Methods: Totally 33 patients undergoing elective transsphenoidal pituitary adenoma resection were randomly divided into 2 groups, 16 in sevoflurane group and 17 in enflurane group. Inhaled sevoflurane or enflurane, adjusted to the concentration of 0.5, 1.0, 1.5 minimum alveolar concentration (MAC), ICP, CPP, mean arterial pressure (MAP) and heart rate (HR). Results: Both sevoflurane and enflurane induced ICP increased, CPP decreased, and MAP and HR decreased, both of which were positively correlated with inhaled concentration. Enflurane was more changed than sevoflurane. Conclusion: The changes of ICP, CPP and MAP caused by sevoflurane and enflurane are within the normal range, and the change range of sevoflurane is smaller than that of enflurane. Tip sevoflurane is more suitable for use in cranial anesthesia.
