目的 对采用粗通道髓芯减压术联合骨诱导活性材料治疗早期股骨头坏死的远期效果进行分析探讨.方法 采用粗通道髓芯减压术联合骨诱导活性材料治疗早期ONFH 40例50髋, 其中男28例, 女12例, 平均年龄28.3 (20~40) 岁.左侧40髋, 右侧10髋.ARCOI-A、I-B、I-C (坏死面积小于股骨头总面积40%者) 期分别为21、15、14髋.术后从临床结果和X线表现评价总的疗效.结果 所有患者髋部疼痛于术后消失, 关节活动功能明显改善, 无血管神经损伤和感染等并发症.术后末次随访时间平均10.5年 (10~12年), 优40髋, 良7髋, 可3髋, 差0髋, 总优良率为94.0%.其中, I-A期中优18髋, 良2髋, 可1髋, 差0髋, 优良率为95.2%.I-B期中优12髋, 良2髋, 可1髋, 差0髋, 优良率为93.3%.I-C期中优10髋, 良3髋, 可1髋, 差0髋, 优良率为92.9%.各期优良率之间相比较, 差别无统计学意义, P>0.05.结论 粗通道髓芯减压术联合骨诱导活性材料治疗ARCOI期ONFH的远期效果优良, 尤其适用于年轻患者.“,”Objective To analyze the long-term effects of core decompression with thick passage combined osteoinduction active meterial to treat early osteonecrosis of femoral head (ONFH). Methods A total of 40 cases with 50 hips suffered from the early stage ONFH underwent the surgery of core decompression with thick passage combined osteoinduction active meterial, male of 28 cases, female of 12 cases, the mean age was 28. 3 (20 ~ 40) years old. There were 40 hips in the left side and 10 hips in the right side. There were 21、15、14 hips in stage ARCO I-A, I-B and I-C (necrotic area was less than 40% of the total area of the femoral head) respectively. The clinical effect and X-ray performance were used to evaluate postoperative effects of the cases. Results The pain of the hips disappeared in all patients after the operation, the function of hip joint activity was significantly improved, without the complications about injury of vascular and nerve as well as infection. The Patients were followed up for an average of 10. 5 years (10 to 12 years) at the last time after the operation. There were the excellent in 40 hips, good in 7 hips, fair in 3 hips, poor in 0 hip, the total rate of excellent and good was 94. 0%. There were excellent in 18 hips, good in 2 hips, fair in 1 hip, poor in 0 hip in stage I-A. There were excellent in 12 hips, good in 2 hips, fair in 1 hip, poor in 0 hip in stage I-B. There were excellent in 10 hips, good in 3 hips, fair in 1 hip, poor in 0 hip in stage I-C. The rate of excellent and good results was95. 2%, 93. 3% and 92. 9% in stage I-A, I-B and I-C respectively. There was no statistically significant difference between excellent and good rates in each stages, I-A, I-B and I-C, P> 0. 05. Conclusion The long-term effects of the core decompression with thick passage combined osteoinduction active meterial to treat the osteonecrosis of the femoral head in ARCO I stage are excellent and good, especially suitable for young patients.
