螺旋CT导向的立体定向开放直视手术切除脑内小病灶

目的总结螺旋CT导向的立体定向开放手术切除脑内小病灶的经验体会和技术要点。方法应用CRW头颅立体定向系统及philip螺旋CT头颅影像三维重建系统,将立体定向技术与开放手术相结合,治疗23例直径<2.5 cm脑内病灶,临床症状以癫rn rnrn发作为主的患者。结果随访3个月至2年,6例术后癫 rnrn未再发作,14例发作次数及程度明显减少,其中发作次数减少为术前50%的10例,无手术并发症发生。结论对于CT或MR检查显示的界限清楚的,最大直径<2.5 cm,临床症状以癫 rnrn发作为主的脑深部小病灶,CT导向的立体定向开放手术是一种简便,准确,微侵袭,安全且疗效满意的手术方法。利用螺旋CT三维重建功能及图像分析软件确定病灶中心点,使其误差<0.5 mm。避开脑重要功能区,在定向仪探针引导下寻找并切除病灶。
