新生儿休克时酸碱紊乱及阴离子间隙分析

总结２６例新生儿休克病人的血气分析，电解质及阴离子间隙的变化后指出：新生儿休克轻度者多为单纯性代谢性酸中毒，中、重度休克常存在二重酸碱紊乱，多为代酸合并呼酸。休克程度愈重发生高ＡＧ酸中毒的比例愈大，提出高ＡＧ酸中毒病人应避免纠酸过量转为代谢性碱中毒成为更复杂的三重酸碱紊乱，顽固性酸中毒应注意纠正低氧血症、原发病治疗，热量供应减少乳酸血症及丙酮酸血症，注意肾功能，合并呼吸衰竭及时呼吸器治疗以提高新生儿休克的治愈率。
