[目的]了解结核病控制中医疗机构与结防机构的合作现状、程度及存在问题,探寻、完善适应可持续发展要求的医防合作新思路。[方法]对2005~2008年长沙市芙蓉区结核病人报告、转诊、到位、管理等方面的资料进行分析。[结果]2005~2008年结防门诊累计登记初诊结核病病人1295例,其中就诊的占51.51%,转诊、追踪的占45.17%。非结防机构合计报告结核病病人2516例,其中定点医院报告的占49.44%,综合医院报告的50.56%;定点医院病人转诊率为9.13%,转诊到位率为91.07%,结防机构登记管理率为29.18%;综合医院转诊率为46.37%,转诊到位率为40.50%;结防机构追踪率为94.40%,追踪到位率为19.90%;总体到位率为29.42%。可疑症状者推荐102例,推荐到位率为38.24%。[结论]芙蓉区结核病控制项目的医防合作存在较大缺陷,实施效果欠佳。
[Objective] To understand the status quo, extent and existing problems of cooperation between medical institutions in tuberculosis control and prevention and treatment institutions, explore and improve new ideas of medical cooperation in adapting to the requirements of sustainable development. [Methods] The data of tuberculosis patients report, referral, place and management in Furong District of Changsha City from 2005 to 2008 were analyzed. [Results] A total of 1295 newly diagnosed tuberculosis patients were enrolled in the end of 2006-2008, of which 51.51% were referred to the TB clinic and 45.17% were referred and traced. Non-stop agencies reported a total of 2516 cases of tuberculosis patients, of which 49.44% were reported by designated hospitals and 50.56% reported by general hospitals; the referral rate of patients in designated hospitals was 9.13% and the referral rate was 91.07% The rate of referral to general hospitals was 46.37% and the referral rate was 40.50%. The follow-up rate of the prevention and treatment institutions was 94.40% and the follow-up rate was 19.90%. The overall in-place rate was 29.42%. Suspicious symptoms were recommended in 102 cases, the recommended rate was 38.24%. [Conclusion] There was a big flaw in the medical cooperation between TB control project in Furong District and its implementation was not good.
