目的:结合手术部位探讨3种不同填塞材料(凡士林纱条、膨胀海绵、胶原蛋白止血海绵/明胶海绵+膨胀海绵)对功能性鼻窦鼻内窥镜术(FESS)术后病人出血情况的影响。方法:对2008年3月至2011年3月在我院行FESS术后患者291例(鼻中隔偏曲矫正、鼻甲部分切除及鼻窦手术患者),按不同手术部位及填塞材料分别分为3组,统计术后及取出填塞物时出血情况,比较不同手术部位、填塞材料下出血率。结果:鼻中隔偏曲矫正及鼻甲部分切除患者术后出血率高,膨胀海绵填塞较凡士林纱条填塞术后出血率低,凡士林填塞术后出血率最高。结论:手术部位与填塞材料同时影响鼻内镜术后出血率,鼻中隔偏曲矫正及鼻甲部分切除患者出血率受填塞材料影响最大,膨胀海绵填塞止血效果最好。
OBJECTIVE: To investigate the effect of three different packing materials (Vaseline, swollen sponge, collagen hemostatic sponge / gelatin sponge + swollen sponge) on the bleeding of patients after functional sinusitis and nasal endoscopy (FESS). Methods: From March 2008 to March 2011 in our hospital, 291 patients undergoing FESS surgery (nasal septum deviation correction, partial nasotomy and sinus surgery) were divided into three groups according to different operative sites and stuffing materials, Statistics postoperative bleeding and remove the stuffing situation, compare different surgical sites, bleeding rate under the stuffing material. Results: The postoperative bleeding rate was higher in patients with nasal septum deviation correction and partial nasal turbinatesctomy. The rate of bleeding after sponge packing with vaseline gauze was lower than that with vaseline gauze, and the bleeding rate after vaseline packing was the highest. CONCLUSIONS: The site of operation and filling material affect the rate of hemorrhage after endoscopic sinus surgery. The hemorrhage rate of nasal septum deviation correction and partial nasal turbinates is the most affected by the stuffing material.
