男,41岁,船员,因足癣口服上海产灰黄霉素5片,约10分钟后即感心烦、头昏,口干、胃脘部烧灼感,继而发生全身性皮肤潮红及红肿,以头面部为著,双眼睑红肿致不能睁开。服药约12小时就诊。确诊为灰黄霉素过敏反应,给予强的松及抗过敏治疗,5天后全身症状恢复正常。【讨论】灰黄霉素为抗浅表真菌的抗生素,主要口服给药,但易发生恶心、食减等胃肠道反应。约3％的病人发生皮疹、红斑等过敏反应,停药后可消退,本例经过与上述相符,偶有发生血管神经性水肿或持继性荨麻疹。本品与青霉素之间可能有交叉过敏反应。
