目的 :研究急性脑梗死患者D-二聚体与血浆B型脑钠肽前体(NT-pro BNP)水平对患者预后的影响。方法 :临床纳入80例我院2013年3月~2015年3月期间收治的急性脑梗死患者作为脑梗死组,另选取同期80例来我院体检中心健康检查的正常人作为对照组,观察两组对象D-二聚体以及NT-pro BNP水平。将脑梗死组患者根据脑梗死面积分为大面积脑梗组(n=32)与小面积脑梗组(n=48),观察两组不同脑梗死面积患者的D-二聚体、NT-pro BNP水平以及神经功能。比较存活与死亡病例入院时相关指标水平,观察D-二聚体与NT-pro BNP水平对预后的影响。结果 :脑梗死组患者D-二聚体以及NT-pro BNP水平均明显高于对照组。大面积脑梗组患者D-二聚体、NT-pro BNP水平以及NIHSS评分均明显高于对照组。对患者随访半年,死亡患者入院时候D-二聚体、NTpro BNP水平均远高于生存的患者。结论 :急性脑梗死患者D-二聚体以及NT-pro BNP水平明显升高,且脑梗死面积越大,D-二聚体、NT-pro BNP水平越高,患者预后效果越差。D-二聚体以及NT-pro BNP可作为观察急性脑梗死患者预后判断的临床指标。
Objective: To study the effect of D-dimer and plasma B-type natriuretic peptide (BNP) level on the prognosis of patients with acute cerebral infarction. Methods: Eighty patients with acute cerebral infarction who were admitted to our hospital from March 2013 to March 2015 were enrolled as cerebral infarction group. Another 80 healthy individuals from our hospital during the same period were selected as control group. Group D-dimer and NT-pro BNP levels. Patients in cerebral infarction group were divided into two groups according to infarction area: large area cerebral infarction group (n = 32) and small area cerebral infarction group (n = 48). D-dimer and NT- pro BNP levels and neurological function. The relative indexes of survivors and deaths on admission were compared, and the effect of D-dimer and NT-pro BNP on prognosis was observed. Results: The levels of D-dimer and NT-pro BNP in cerebral infarction group were significantly higher than those in control group. D-dimer, NT-pro BNP levels and NIHSS scores were significantly higher in the large area cerebral infarction patients than in the control group. Patients were followed for six months, D-dimer, NTpro BNP levels were significantly higher in survivors than in survivors. Conclusion: The levels of D-dimer and NT-pro BNP in patients with acute cerebral infarction are significantly increased. The larger the infarct size, the higher the levels of D-dimer and NT-pro BNP, the poorer the prognosis of patients. D-dimer and NT-pro BNP can be used as clinical indicators to judge the prognosis of patients with acute cerebral infarction.
