目的通过免疫组化方法研究RECK基因及基质金属蛋白酶-2、(matrix metalloproteinase 2,MMP-2)、基质金属蛋白酶-9、(matrix metalloproteinase 9,MMP-9)在腮腺多形性腺瘤(pleomorphic adenoma,PA)初发、复发及恶变中的表达及其意义。方法收集2004年至今,新疆医科大学第一附属医院颌面外科收治并经手术治疗的腮腺PA标本50例,其中初发36例,恶变7例,复发7例和10例正常腮腺组织(初发组中远离腮腺肿物的正常腮腺组织)。通过免疫组织化学方法检测RECK、MMP-2和MMP-9在各组标本中的表达,并采用半定量方法进行计量。对RECK与MMP-2及MMP-9在各组中的表达采用SPSS 21.0软件包对数据进行统计学分析。结果 MMP-2及MMP-9在正常组与复发组差异有统计学意义(P<0.05),正常组与恶变组差异有统计学意义(P<0.05),初发组与复发组差异有统计学意义(P<0.05),初发组与恶变组差异有统计学意义(P<0.05)。RECK与MMP-2、MMP-9在各个组的表达都呈负相关(r=-0.887,P<0.05;r=-0.844,P<0.05)。结论 RECK基因与MMP-2及MMP-9在腮腺PA复发、恶变中的表达呈均呈负相关,RECK基因可能通过调控MMP-2及MMP-9参与PA的浸润、复发和恶变的过程,这对于判别肿瘤侵蚀性和预后指标是极有可能。
Objective To investigate the expression of RECK gene and the expression of matrix metalloproteinase 2 (MMP-2), matrix metalloproteinase 9 (MMP-9) in pleomorphic adenoma , PA) in primary, recurrent and malignant transformation and its significance. Methods Totally 50 parotid PA specimens were collected from the Department of Maxillofacial Surgery, the First Affiliated Hospital of Xinjiang Medical University from 2004 till now. Among them, 36 cases were newly diagnosed, 7 cases were malignant, 7 cases recurred and 10 cases were normal parotid gland Normal parotid tissue away from the parotid gland in the group). The expression of RECK, MMP-2 and MMP-9 in each group of specimens was detected by immunohistochemical method and quantified by semi-quantitative method. The data of RECK, MMP-2 and MMP-9 in each group were analyzed by SPSS 21.0 software package. Results The difference of MMP-2 and MMP-9 between the normal group and the recurrent group was statistically significant (P <0.05), the difference between the normal group and the malignant group was statistically significant (P <0.05), the difference between the initial group and the recurrent group was statistically Significance (P <0.05), the difference between the initial group and malignant group was statistically significant (P <0.05). RECK was negatively correlated with MMP-2 and MMP-9 in all groups (r = -0.887, P <0.05; r = -0.844, P <0.05). Conclusions RECK gene has a negative correlation with the expression of MMP-2 and MMP-9 in PA recurrence and malignant transformation of parotid gland. RECK gene may participate in the infiltration, recurrence and malignant transformation of PA by regulating MMP-2 and MMP-9 For discriminating tumor aggressiveness and prognostic indicators is most likely.
