中药是中医治疗疾病的重要材料,其使用的准确性对患者治疗的效果具有决定性作用。然而,笔者在临床工作中发现,我院中药房在调配中药的过程中存在两大类问题:一类是中医师在书写处方时出现的问题,如处方中的药物配伍不合理、剂量不准确或脚注错误等。另一类是中药师在调配药物过程中出现的问题,如药物之间替代不妥当、抓错药物等。笔者认为,我院只有规范中药处方的书写方式和调配程序,提高中医师和中药师的工作责任感,在调配中药处方时做好核对工作,才能有效地解决上述问题,避免医疗纠纷的发生。
Traditional Chinese medicine is an important material for the treatment of diseases in Chinese medicine. The accuracy of its use plays a decisive role in the treatment of patients. However, in clinical work, the author found that there are two major types of problems in the process of Chinese medicine pharmacy deployment in our hospital. One is the problems that Chinese medicine practitioners have when writing prescriptions. For example, the compatibility of medicines in the prescription is not reasonable and the dosage is not accurate Or footnote error and so on. The other is the traditional Chinese medicine practitioners in the deployment of drugs in the process of problems, such as improper replacement of drugs, catch the wrong drugs. The author believes that only by regulating the way of writing prescriptions and formulating prescriptions for Chinese medicine prescriptions in our hospital can we improve the sense of responsibility of Chinese medicine practitioners and Chinese medicine practitioners and do a good job of reconciliation when formulating traditional Chinese medicine prescriptions so as to effectively solve the above problems and avoid medical disputes.
