目的应用优化的基于体素的形态测量学(VBM)技术,分析HIV阳性男性患者全脑灰质异常情况。方法选取47例HIV阳性男性受试者和与之年龄、性别匹配的23名HIV阴性者(正常对照组)进行磁共振3D T1WI扫描。利用基于统计参数图(SPM)8软件的VBM 8工具箱对扫描获得的图像进行预处理,对HIV患者和正常对照组的全脑灰质体积采用t检验进行比较,找出灰质体积差异区域的改变。结果应用VBM技术,发现患者组的左侧三角部额下回、左侧眶部额中回、左侧颞上回和双侧前扣带回较对照组减小(P<0.05)。中脑导水管周围灰质及侧脑室周围灰质较对照组增大。结论左侧三角部额下回、左侧眶部额中回、左侧颞上回和前扣带回灰质体积减小可能是评价HIV患者神经认知障碍的一项客观指标。
Objective To analyze the abnormalities of whole brain gray matter in HIV-positive men with optimized voxel-based morphometric (VBM) technique. Methods Magnetic resonance 3D T1-weighted MRI was performed in 47 HIV-positive male subjects and 23 HIV-negative persons (normal control group) matched with their age and sex. Scanned images were preprocessed using the VBM 8 toolbox based on statistical parametric map (SPM) 8 software to compare the global gray matter volume of HIV patients with that of the normal control group using t-test to find changes in gray matter volume differences . Results Using VBM technique, we found that patients in the left inferior frontal triangle, frontal gyrus of the left orbit, left superior temporal gyrus and bilateral anterior cingulate gyrus decreased compared with the control group (P <0.05). The gray matter in periaqueductal gray and the periventricular gray matter increased compared with the control group. Conclusions The lower left triangle, the middle frontal gyrus of the left orbit, the left superior temporal gyrus and the anterior cingulate gray volume may be objective indicators for evaluating neurocognitive impairment in HIV patients.
