本文报告任性胰腺炎所致梗阻性黄疸３３例，术前误诊２０例，（术中仍误诊胰头癌８例），１３例诊断为慢性胰腺炎。３３例均经手术治疗，术式的选择：胆总管十二指肠或胆总管空肠Ｒｏｕｘ－ｙ型吻合术（２１例），胆囊空肠Ｒｏｕｘ－ｙ型吻合术（２例），Ｏｄｄｉ氏括约肌切开成形术（２例），胰＋二指肠切除术（８例）。来后３０例痊愈，３例死亡。文中对慢性胰腺炎所致梗阻性黄疸的发病机制、分类分型、诊断及治疗原则进行了讨论。
In this paper, 33 cases of obstructive jaundice caused by perinatal pancreatitis were reported. There were 20 cases misdiagnosed before operation (8 cases were still misdiagnosed as pancreatic head cancer in operation) and 13 cases were diagnosed as chronic pancreatitis. Thirty-three patients were surgically treated. Roux-y type choledochostomy (21 cases), Roux-y type choledochojejunostomy (2 cases), oddi’s sphincterotomy Open surgery (2 cases), pancreatic + duodenal resection (8 cases). Thirty patients recovered and 3 died. The pathogenesis, classification, diagnosis and treatment of obstructive jaundice caused by chronic pancreatitis are discussed in this article.
