目的:提高临床对皮损不典型的手足口病的认识。方法:回顾性分析149例手足口病(hand-foot-mouth disease,HFMD)住院患儿的临床资料,根据皮损特点,将HFMD分为典型、不典型Ⅰ型及不典型Ⅱ型,分别比较不典型Ⅰ型、Ⅱ型各自与典型HFMD皮损特征及重症率的差异。结果:不典型Ⅰ型出疹部位少,甚至单一,以红斑、丘疹、结痂为主,水疱罕见,皮损相对小而少,病情较重。不典型Ⅱ型皮损范围广泛,以红斑、丘疹、水疱和结痂为主,还常见高出皮面的透明水疱,皮损相对大而多,病情较轻。结论:早期识别不典型HFMD的皮损特征为及时诊断不典型HFMD及预测其病情发展提供了重要的临床依据。
Objective: To improve clinical understanding of atypical hand, foot and mouth disease. Methods: The clinical data of 149 hospitalized children with hand-foot-mouth disease (HFMD) were retrospectively analyzed. According to the characteristics of skin lesions, HFMD was divided into typical, atypical type I and atypical type II, respectively The difference between the characteristic of atypical type Ⅰ and type Ⅱ and the typical lesions of HFMD and the severity of severe. Results: The atypical type Ⅰ had few or even a single rash, with erythema, papules and crusts as the main ones. The blisters were rare, the lesions were relatively small and few, and the disease was serious. Atypical type Ⅱ lesions a wide range of erythema, papules, blisters and scab the main, but also common above the leather transparent blisters, lesions are relatively large and more, less serious condition. CONCLUSION: Early identification of atypical HFMD lesions provides an important clinical basis for timely diagnosis of atypical HFMD and prediction of its progression.
