目的观察高效抗反转录病毒治疗(highly active antiretroviral therapy,HAART)联合中药治疗艾滋病疗效。方法回顾分析南宁市第四人民医院2009年6月-2013年6月收治的200例符合入组标准的艾滋病患者作为观察对象,其中观察组100例,对照组100例。对照组单纯采用HAART治疗(齐多夫定300 mg+拉米夫定150 mg+奈韦拉平200 mg,2次/d),观察组在对照组的基础上给予扶正解毒方中药治疗,早晚服用。两组治疗30 d为1个疗程,均治疗6个疗程。观察患者症状体征改善情况,治疗前后采用流式细胞计数测定CD4+,采用RT-PCR检测病毒载量。将数据录入SPSS 17.0软件进行统计分析,采用t检验或χ2检验,P<0.05为差异有统计学意义。结果观察组乏力、肌关节疼痛、皮肤瘙痒、气短等治疗效果明显优于对照组,差异有统计学意义(均P<0.05)。观察组和对照组治疗后CD4+绝对计数较治疗前升高(均P<0.05),差异有统计学意义;观察组治疗后CD4+绝对计数高于对照组(P<0.05),差异有统计学意义。观察组治疗后病毒载量较治疗前降低,差异有统计学意义(P<0.05);对照组治疗前后病毒载量差异无统计学意义(P>0.05)。结论 HAART联合扶正解毒中药能够提高CD4+数量,促进免疫重建,降低病毒载量,可在临床积极推广。
Objective To observe the efficacy of highly active antiretroviral therapy (HAART) combined with traditional Chinese medicine in the treatment of AIDS. Methods A retrospective analysis of Nanning Fourth People’s Hospital from June 2009 to June 2013 admitted 200 cases of AIDS patients meet the inclusion criteria as the observation object, the observation group of 100 cases, control group of 100 cases. The control group was treated with HAART alone (zidovudine 300 mg plus lamivudine 150 mg plus nevirapine 200 mg twice daily). The observation group was given Fuzheng Jiedu Decoction on the basis of the control group, taking it morning and evening. Two groups of 30 days for a course of treatment, are treated 6 courses. To observe the improvement of symptoms and signs of patients before and after treatment using flow cytometry to determine CD4 +, using RT-PCR detection of viral load. The data were entered SPSS 17.0 software for statistical analysis, using t test or χ2 test, P <0.05 for the difference was statistically significant. Results The observation group was significantly less effective than the control group in terms of fatigue, muscle and joint pain, pruritus and shortness of breath. The difference was statistically significant (both P <0.05). The absolute CD4 + counts in observation group and control group were higher than those before treatment (all P <0.05), and the difference was statistically significant. CD4 + absolute count in observation group was higher than that in control group (P <0.05), and the difference was statistically significant . The viral load in the observation group was lower than that before treatment, the difference was statistically significant (P <0.05). There was no significant difference in the viral load in the control group before and after treatment (P> 0.05). Conclusions HAART combined with Fuzheng detoxification traditional Chinese medicine can increase the number of CD4 +, promote immune reconstitution and reduce the viral load, which can be actively promoted in clinic.
