[目的]探讨急性心肌梗死(AMI)患者经皮冠状动脉介入术(PCI)前后脂联素(APN)动态变化与左心室收缩功能的关系.[方法]选择2015年1月至2016年1月于本院行择期PCI治疗的AMI患者100例作为观察组,选取性别、年龄匹配的体检健康者90例作为对照组.观察组于PCI术前、术后1个月、术后3个月及对照组检测A PN水平,并采用二维斑点追踪成像技术检测观察组左心室收缩功能[左室舒张末期容积(LVESV)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)],并分析APN与LVESV、LVESD、LVEF的相关性.[结果]观察组术前及术后1、3个月APN低于对照组,差异有统计学意义(P<00.5);观察组术后1、3个月APN高于术前(P<00.5),且术后3个月高于术前1个月,差异有统计学意义(P<00.5).观察组患者术后1、3个月LVESV、LVESD低于术前,LVEF高于术前(P<00.5),且术后3个月LVESV、LVESD低于术后1个月,LVEF高于术后1个月,差异有统计学意义(P<00.5).Pearson相关分析显示,血清APN与LVESV、LVESD呈负相关(r=-07.16、-06.18,P<00.5),与LVEF呈正相关关系(r=06.89,P<00.5).[结论]AMI患者PCI后APN水平与左心室收缩功能变化密切相关.“,”[Objective]To explore the relationship between the adiponectin (APN ) dynamic changes and left ventricular systolic function in patients with acute myocardial infarction (AMI) before and after percutane-ous coronary intervention (PCI) .[Methods] A total of 100 cases of AMI patients treated with elective PCI in our hospital from January 2015 to January 2016 were selected as the observation group ,and 90 healthy exam-iners with the matching gender and age were selected as the control group .The levels of APN were detected in the both group before PCI as well as 1 month and 3 months after operation .The two-dimensional speckle tracking imaging was used to detect the left ventricular systolic function [left ventricular end diastolic volume (LVESV) ,left ventricular end systolic diameter (LVESD) and left ventricular ejection fraction (LVEF)] of the observation group .The correlation between APN and LVESV ,LVESD ,LVEF was analyzed .[Results]The APN level before operation ,1 month and 3 months after operation were lower in the observation group than those in the control group ( P <00.5) .The APN level of the observation group was higher at 1 month and 3 months after operation than it was before operation ( P <00.5) ,and the APN level at 3 months after operation was higher than that at 1 month before operation ( P <00.5) .The levels of LVESV and LVESD at 1 month or 3 months after operation in the observation group were lower than those before operation ,while the LVEF level was higher than that before operation ( P < 0 .05) .The levels of LVESV and LVESD at 3 months after operation were lower than those at 1 month after operation ,while the LVEF level was higher than that at 1 month after operation ( P <00.5) .Pearson correlation analysis showed that serum APN was negatively correlated with the LVESV and LVESD (r= -07.16 ,-06.18 ,P <00.5) ,and positively correla-ted with LVEF (r=06.89 ,P < 0 .05) .[Conclusion] APN level of AMI patients after PCI is closely related with left ventricular systolic function .
