目的探讨比较单纯性肥胖儿童及正常儿童血清瘦素、生长激素(growth hormone GH)生物、免疫活性及瘦素水平,为临床进一步明确儿童单纯性肥胖的病因提供新的方向。方法随机选取单纯性肥胖儿童68例(单纯性肥胖组)和正常儿童72例(正常对照组),均空腹抽取静脉血离心,包被试管放射免疫法定测定瘦素水平,BAF/GM细胞株法测定血清GH生物活性,免疫放射分析法(IRMA)测定血清人GH的免疫活性,并评估两组的B/I比值。结果单纯性肥胖组血清样本GH生物活性和免疫活性的平均值分别为(1.26±0.50)、(2.24±0.72),正常对照组GH生物活性和免疫活性分别为(2.18±0.77)、(2.07±0.66),两组生长激素生物活性与免疫活性比较差异无统计学意义(P>0.05)。正常对照组血清样本的B/I比值平均为(1.02±0.21),单纯性肥胖组为(0.53±0.13),两组的B/I比值比较差异有统计学意义(P<0.05)。单纯性肥胖组血清瘦素含量为(23.58±4.35),正常对照组血清瘦素含量为(6.95±3.61)。单纯性肥胖组血清瘦素水平高于正常对照组(P<0.05)。结论单纯性肥胖儿童瘦素水平较正常儿童增高,可能存在瘦素抵抗现象。单纯性肥胖儿童与正常儿童比较生长激素生物活性明显减低。
Objective To compare the levels of serum leptin, growth hormone GH, immunological activity and leptin in simple obesity children and normal children, and to provide a new direction for clinical further clarifying the cause of childhood simple obesity. Methods 68 cases of simple obesity children (simple obesity group) and 72 normal children (normal control group) were randomly selected, both were centrifuged by fasting venous blood and the contents of leptin were determined by radioimmunoassay in vitro. BAF / GM cell line Serum GH bioactivity was measured, immunological radioimmunoassay (IRMA) was used to measure the immunosuppressive activity of serum human GH, and the B / I ratio of both groups was assessed. Results The average GH activity and immunological activity of serum samples from simple obesity group were (1.26 ± 0.50) and (2.24 ± 0.72), respectively. The GH activity and immune activity of normal group were (2.18 ± 0.77) and (2.07 ± 0.66). There was no significant difference between the two groups in growth hormone bioactivity and immune activity (P> 0.05). The B / I ratio of the normal control group was (1.02 ± 0.21) and that of the simple obesity group (0.53 ± 0.13), respectively. The B / I ratio of the two groups had significant difference (P <0.05). Serum leptin level was (23.58 ± 4.35) in simple obesity group and (6.95 ± 3.61) in normal control group. Serum leptin level in simple obesity group was higher than that in normal control group (P <0.05). Conclusions The leptin level in simple obese children is higher than that in normal children, so there may be leptin resistance. Compared with normal children, simple obesity children significantly reduced the biological activity of growth hormone.
