目的 探讨非洛地平对老年高血压伴肾功能损害时的降压疗效。方法 选择了近 3年来住院的原发性高血压病人伴轻、中度肾功能损害 1 48例。入选病人入院前 3 d停用各种降压药 ,3 d后 ,测量病人 8:0 0时和 2 0 :0 0时血压各 1次 ,取其平均值作为治疗前基础血压。次日 ,投给非洛地平缓释片 ,每日 5~ 1 0 mg,晨 8时顿服 ,然后用上述方法 ,测量服药后第 1 ,2 ,4周当天血压 ,并与治疗前进行对比分析。结果 治疗后收缩压于第 1周即出现效果 ,统计学处理已有显著性差异 (P <0 .0 5 )。治疗至第 2、第 4周时 ,收缩压下降明显、平稳 (均为 P <0 .0 1 )。而舒张压下降较缓慢 ,治疗第 1周后 ,总有效率仅为 48.6 % ,于第 2周时疗效出现 ,至第 4周时 ,血压一直稳定于正常水平 ,总有效率达 81 .1 %。结论 非洛地平缓释片口服后 ,通过选择性的扩张外周血管而达到降压作用。临床观察中 ,没有发现非洛地平对肾功能有进一步损害。表明当老年高血压病人伴有肾功能损害时 ,非洛地平可作为一种安全、可靠的一线降压药
Objective To investigate the antihypertensive effects of felodipine on senile hypertension with renal dysfunction. Methods One hundred and forty-eight patients with essential hypertension who were hospitalized in the past three years with mild or moderate renal dysfunction were selected. Patients were selected 3 d before admission to disable a variety of antihypertensive drugs, 3 d after the measurement of the patient 8:00:00 and 20:00 blood pressure 1 times, whichever is the average value as the baseline blood pressure before treatment. The next day, cast sustained-release tablets of felodipine daily 5 ~ 10 mg Dayton 8 suits, and then use the above method, measured after taking the first two weeks, four weeks of the day blood pressure and compared with before treatment analysis. Results After treatment, systolic blood pressure appeared in the first week, and the effect was statistically significant (P <0.05). Systolic blood pressure decreased significantly and steadily (both P <0.01) after the second and fourth week of treatment. The diastolic blood pressure decreased more slowly, the first week after treatment, the total effective rate was only 48.6%, the second week of efficacy, the fourth week, the blood pressure has been stable at normal levels, the total effective rate was 81.1% . Conclusion After oral administration of felodipine sustained-release tablets, blood pressure can be reduced by selective expansion of peripheral blood vessels. In clinical observation, felodipine did not find any further damage to renal function. That when elderly hypertensive patients with renal impairment, felodipine can be used as a safe, reliable first-line antihypertensive drugs
