目的探讨转化生长因子β1(TGF-β1)和胰岛素样生长因子-Ⅱ(IGF-Ⅱ)在原发性肝细胞癌中的表达及其与肝癌转移的关系。方法采用常规SP免疫组织化学染色检测39例原发性肝细胞癌患者标本中TGF-β1和IGF-Ⅱ的表达,并以21例肝硬化患者、16例肝外伤及肝囊肿患者标本作为对照,分析其表达水平与肝癌转移的关系。结果 TGF-β1和IGF-Ⅱ在转移组肝癌组织的表达率均明显高于无转移组,在低分化组的阳性表达率明显高于高分化组。TGF-β1在正常肝脏组织及肝硬化组织中几乎不表达。IGF-Ⅱ在正常肝脏组织及肝硬化组织中呈弱阳性表达或几乎不表达。TGF-β1和IGF-Ⅱ在肝癌组织中的表达水平呈正相关。结论 TGF-β1和IGF-Ⅱ的表达与肿瘤分化程度及侵袭转移的发生有关,可作为估计预后的指标,对判断肝癌的恶性程度,预测生物学行为具有一定的临床意义。
Objective To investigate the expression of transforming growth factor β1 (TGF-β1) and insulin-like growth factor-Ⅱ (IGF-Ⅱ) in primary hepatocellular carcinoma (HCC) and their relationship with metastasis of hepatocellular carcinoma. Methods The expression of TGF-β1 and IGF-Ⅱ in 39 specimens of primary hepatocellular carcinoma was detected by routine SP immunohistochemical staining. The specimens of 21 patients with liver cirrhosis, 16 patients with hepatic trauma and hepatic cysts were used as controls. To analyze the relationship between the expression and the metastasis of liver cancer. Results The positive rates of TGF-β1 and IGF-Ⅱ in metastatic hepatocellular carcinoma tissues were significantly higher than those in non-metastatic carcinoma groups. The positive rates of TGF-β1 and IGF-Ⅱ were significantly higher in poorly differentiated group than those in well-differentiated group. TGF-β1 is almost not expressed in normal liver and cirrhosis tissues. IGF-Ⅱ in normal liver tissue and cirrhosis tissues were weakly positive or almost no expression. The expression of TGF-β1 and IGF-Ⅱ in HCC tissues was positively correlated. Conclusion The expression of TGF-β1 and IGF-Ⅱ is related to the degree of tumor differentiation and invasion and metastasis, which can be used as an index to estimate the prognosis. It has certain clinical significance for judging the malignant degree of liver cancer and predicting the biological behavior.
