４０例急性心肌梗塞（以下简称急性心梗）患者于静脉内溶栓前、溶栓２、２４、７２ｈ及１周应用放免法检测血浆心钠素（ＡＮＰ）、肾素（ＰＲＡ）及血管紧张素Ⅱ（ＡｎｇⅡ）水平，２０例健康人为对照组。发现溶栓前心梗组较对照组ＡＮＰ明显升高（Ｐ＜０．０１），２ｈ达高峰（Ｐ＜０．００１），２４、７２ｈ及１周有明显下降趋势，但仍较对照组增高（Ｐ均＜０．０５）。其中２６例再灌注指标明确者ＡＮＰ２ｈ较溶栓前明显升高（Ｐ＜０．００１）。１４例无再灌注指标者则无显著性差异。心梗组ＡｎｇⅡ于２ｈ达高峰，与对照组有显著性差异（Ｐ＜０．０１），２４、７２ｈ及１周逐渐下降。ＰＲＡ于２、２４及７２ｈ亦均较对照组升高（Ｐ＜０．０５及０．０１）。不同部位及不同心功能状态的心梗患者血浆ＡＮＰ水平无显著性差异。＊Ｐ＜０．００１心梗组ＡＮＰ水平溶栓前、溶栓２４、７２ｈ及１周与溶栓２ｈ比：ΔＰ＜０．０１；ΔΔＰ＜０．００１２．２有再灌注指征及无再灌注指征者溶栓治疗后２ｈ血浆ＡＮＰ水平的变化见表２．２６例有再灌注指征者均发生了再灌注心律失常，且于溶栓治疗后２ｈＡＮＰ明显升高，与溶栓前ＡＮＰ水平有明显差异（Ｐ＜０．００１）。１４例无再灌注指征者溶栓治疗后２ｈ?
Atrial natriuretic peptide (ANP), renin (PRA), and vascular tension were measured by radioimmunoassay in 40 patients with acute myocardial infarction (AMI) before thrombolysis, Ang Ⅱ level and 20 healthy controls. The levels of ANP in myocardial infarction group before thrombolytic therapy were significantly higher than those in control group (P <0.01), peaked at 2h (P <0.001), but decreased significantly at 24, 72 and 1 week (P <0.05). Among them, ANP2h was significantly higher in 26 cases with reperfusion index than before thrombolysis (P <0.001). There were no significant differences in 14 cases without reperfusion index. Ang Ⅱ peaked at 2h in MI group, which was significantly different from that in control group (P <0.01), and gradually decreased at 24, 72h and 1 week. PRA also increased at 2, 24, and 72 hours (P <0.05 and 0.01).
