目的分析镇巴县活动性肺结核病患者治疗转归及其影响因素,改进防控技术策略与措施。方法收集2014-2015年在镇巴县人民医院结核病专科门诊确诊的476例活动性肺结核病患者,统计分析患者治疗转归以及性别、年龄、初复治、有无合并症、患者管理方式等因素对治疗转归的影响。用SPSS18.0软件进行统计学分析,率和构成比用相对数表示,率之间的比较用卡方检验,以P<0.05为差异有统计学意义,均数间的比较用u检验,检验水准α=0.05。结果 476肺结核病患者中,利福平敏感患者110例,治愈95例,治愈率为86.4%;366例涂阴结核病患者中完成疗程316例,治疗成功率86.3%。影响因素分析显示,使用手机管理有利于结核病患者的治疗转归,年龄≥65岁、有结核病史、复治患者和有糖尿病、尘肺病观察对象等合并症者不利于结核病患者的治疗转归。结论镇巴县结核病患者的治疗效果达到85%目标要求,但仍有提升空间,应继续强化各项防控措施的落实。
Objective To analyze the outcome of treatment and its influential factors in patients with active pulmonary tuberculosis in Zhenba County and to improve the strategies and measures for prevention and control. Methods A total of 476 active pulmonary tuberculosis cases diagnosed in Tuberculosis Outpatient Department of Zhenba County People’s Hospital from 2014 to 2015 were collected and statistically analyzed on the outcome of treatment, sex, age, initial retreatment, complication and patient management Impact on treatment outcome. SPSS18.0 software for statistical analysis, the rate and composition than the relative number, said the rate of comparison between the use of chi-square test to P <0.05 for the difference was statistically significant, the average between the u test, test Level α = 0.05. Results Among 476 pulmonary tuberculosis patients, 110 were susceptible to rifampicin, 95 were cured, and the cure rate was 86.4%. Among 366 tuberculous smear-negative tuberculosis patients, 316 were completed and the success rate was 86.3%. The analysis of influencing factors shows that the use of mobile phone management is beneficial to the treatment of patients with tuberculosis, age ≥ 65 years old, with a history of tuberculosis, retreatment patients and patients with diabetes, pneumoconiosis and other complications are not conducive to the treatment outcome of tuberculosis patients. Conclusion The treatment effect of TB patients in Zhenba County reaches the target of 85%, but there is still room for improvement. Implementation of various prevention and control measures should continue to be strengthened.
