目的:研究无功能肾上腺皮质腺癌声像图特征及其与病理结构的相关性。方法:回顾性分析经二维及彩色多普勒超声诊断的并经手术病理证实的14例肾上腺皮质腺癌患者。结果:14例全部经超声与病理对照分析研究表明,等回声组所占比例最大,占43%(6/14);强回声组占29%(4/14);低回声组占14%(2/14);混合回声2例占14%。B超定位诊断符合率为93%(13/14);定性诊断符合率为57%(8/14)。结论:1.肾上腺皮质腺癌的声像图特征为:①肿瘤是以等回声为主的较大肿瘤,本组均大于8.2cm;②肿瘤形态多样,以不规则形及分叶状常见;③彩色多普勒血流显示瘤内及瘤周血流丰富;④肿瘤内多见液化坏死。2.与病理结构的相关性为肿瘤内回声的高低与肿瘤细胞的大小、排列情况、血管纤维组织及坏死等病理结构含量多少有关。
Objective: To study the sonographic features of non-functional adrenocortical adenocarcinoma and its relationship with pathological structure. Methods: A retrospective analysis of 14 cases of adrenocortical adenocarcinoma diagnosed by two-dimensional and color Doppler ultrasonography confirmed by surgery and pathology. Results: All the 14 cases were analyzed by ultrasound and pathology. The results showed that the echogenic group accounted for 43% (6/14), the echogenic group (29%) and the hypoechogenic group (14%) 2/14); mixed echo in 2 cases accounted for 14%. The coincidence rate of B ultrasound diagnosis was 93% (13/14). The coincidence rate of qualitative diagnosis was 57% (8/14). Conclusion: 1. The characteristics of acoustic images of adrenocortical adenocarcinoma were as follows: ① The tumor was a large tumor dominated by equal echoes, all of which were larger than 8.2 cm. ② The tumors varied in shape, common in irregular shape and lobulated form. Pulmonary blood flow in the tumor and peritumoral blood flow rich; ④ more liquefied necrosis in the tumor. 2. Correlation with the pathological structure of the tumor echo levels and tumor cell size, arrangement, vascular tissue and necrosis and other pathological structure content.
