目的评估3种X线容积成像(XVI)系统的辐射剂量和图像质量。方法回顾性分析2007年—2012年间应用3种XVI系统(组1~3)接受检查的126例病人的数据。每组包括42例病人,每例病人均行锥形束计算机体层摄影术(CBCT)、数字减影血管造影(DSA)和数字荧光透视检查(DF)。评估剂量参数,如剂量面积乘积(DAP)、入射表面剂量(SED)和图像质量参数,如霍斯菲尔德单位(HU)、噪声、信噪比(SNR)、对比噪声比(CNR),并采用适当的统计检验方法进行比较。结果近期XVI的平均DAP和SED值较先前CBCT、DSA、DF的低。除了肝动脉,所有测量部位的HU组间差异不显著。各组间的噪声具有显著差异(P<0.05)。关于CNR和SNR,最近的XVI与之前的版本间显示具有更高和显著的差异。定性地讲,与DSA和DF的先后版本间无差异,CBCT的不同版本之间存在显著差异。结论 CBCT、DSA、DF的新一代XVI系统的辐射剂量是降低的。影像噪声明显更低;SNR和CNR较之前的版本更高。新一代XVI系统的技术进步和帧数的减少使剂量明显降低、图像质量提高。要点①X线容积成像系统越来越多地用于介入放射学手术。②与之前的版本相比,更多现代的XVI系统的辐射剂量更低。③而且更现代的XVI系统可提供更高的影像质量。④技术进步可降低辐射剂量并提高影像质量。
Objective To assess the radiation dose and image quality of three X-ray volumetric imaging (XVI) systems. Methods The data of 126 patients who underwent three types of XVI system (group 1 ~ 3) during 2007-2012 were analyzed retrospectively. Each group consisted of 42 patients, and each patient underwent cone beam computed tomography (CBCT), digital subtraction angiography (DSA) and digital fluoroscopy (DF). The dose parameters such as dose area product (DAP), incident surface dose (SED) and image quality parameters such as the Horsfield unit (HU), noise, signal to noise ratio (SNR), contrast to noise ratio (CNR) Appropriate statistical test methods for comparison. Results The average DAP and SED values of XVI were lower than those of the previous CBCT, DSA and DF. In addition to the hepatic artery, there was no significant difference between the HU groups at all measurement sites. There was a significant difference in noise among groups (P <0.05). With regard to CNR and SNR, recent XVI showed higher and significant differences from previous versions. Qualitatively, there is no difference between prior versions of DSA and DF, and significant differences exist between different versions of CBCT. Conclusion The radiation dose of the new generation of XVI systems of CBCT, DSA, DF is reduced. Image noise is significantly lower; SNR and CNR are higher than previous versions. The new generation of XVI system technology advances and reduce the number of frames significantly reduced dose, improve image quality.
