目的 :对三种方案治疗急性有机磷农药中毒进行临床分析 ,旨在探讨理想治疗方案。方法 :将我科近 12年收治的、中毒前无严重心、肺、肝、肾疾患的急性有机磷农药中毒34 4例作为分析对象 ,其中近 8年收治的 2 37例随机分成改良方案组 (甲组 ) 12 1例与解磷注射液组 (乙组 ) 116例 ,最初 4年收治的 10 7例为传统疗法组 (丙组 ) ,对三组的治愈率、住院时间、阿托品用量等进行比较。结果 :(1)治愈率 :在轻度中度时 ,甲组与乙组无显著差异性 ,乙组显著高于丙组。在中、重度中毒时 ,甲组显著或非常显著地高于乙组 ,乙组非常显著地高于丙组 (P <0 0 0 1)。(2 )住院时间 :甲组非常显著低于乙组 ,乙组非常显著低于丙组 (P <0 0 0 1)。(3)阿托品用量 :甲组非常显著低于丙组 (P <0 0 0 1)。(4)阿托品中毒 :甲组与乙组无显著差异性 ,乙组非常显著低于丙组 (P <0 0 1)。(5 )农药中毒反跳 :甲组非常显著低于乙组 (P <0 0 1) ,乙组显著低于丙组 (P <0 0 5 )。结论 :改良方案分别比传统疗法与解磷注射液疗法具有显著优越性 ,可视为治疗急性有机磷农药中毒理想方案。
Objective: To analyze the clinical efficacy of three kinds of regimens in the treatment of acute organophosphorus pesticide poisoning, and to explore the ideal treatment plan. Methods: A total of 344 cases of acute organophosphorus pesticide poisoning without serious heart, lung, liver and kidney disease admitted to our department in recent 12 years were analyzed. Among them, 2 37 cases were randomly divided into three groups (Group A), 116 cases with group B (12 cases) and Xiefu injection group (group B), and 107 cases received traditional therapy group (group C) in the first 4 years. The cure rate, length of stay, the amount of atropine in the three groups Compare. Results: (1) The cure rate: in mild to moderate, there was no significant difference between group A and group B, group B was significantly higher than group C. In moderate and severe poisoning, group A was significantly or very significantly higher than group B, group B was significantly higher than group C (P <0.01). (2) The duration of hospital stay: Group A was significantly lower than Group B, Group B was significantly lower than Group C (P <0.01). (3) Atropine consumption: Group A was significantly lower than Group C (P <0.01). (4) Atropine poisoning: There was no significant difference between group A and group B, group B was significantly lower than group C (P <0.01). (5) The rebound of pesticide poisoning: Group A was significantly lower than Group B (P <0.01), Group B was significantly lower than Group C (P <0 05). CONCLUSION: The improved protocol has distinct superiority over the traditional therapy and the solution-injection therapy, respectively. It can be regarded as the ideal solution for the treatment of acute organophosphorus pesticide poisoning.
