目的:探讨成人急性白血病中的趋化因子受体-7(CC-chemokine receptor 7 CCR7)表达及其与临床特征的关系。方法:应用流式细胞术检测71例成人急性白血病患者(年龄≥12岁)骨髓白血病细胞CCR7的表达水平,探讨其在成人急性白血病中的表达及与年龄、性别、诊断分型、外周血WBC数、骨髓原始幼稚细胞比例、CD56表达、分子生物学、细胞遗传学,危险度分层、髓外浸润等临床特征的关系。结果:19例急性淋巴细胞白血病(ALL)中CCR7~+者7例(36.8%),52例急性髓系白血病(AML)中CCR7~+者5例(9.6%),在ALL中CCR7~+率明显高于AML中(P<0.05),在ALL中CCR7~+组髓外浸润发生率明显高于CCR7-组(100%vs 41.7%)(P<0.05),ALL伴有髓外浸润组CCR7平均荧光强度(MFI)明显高于无髓外浸润组(50.00±10.42 vs 18.14±1.39)(P<0.05)。结论:CCR7在成人急性白血病中表达增加,在ALL中阳性率高于AML中,并与ALL髓外浸润相关。
Objective: To investigate the expression of CCR7 in adult acute leukemia and its relationship with clinical features. Methods: The expression of CCR7 in myeloid leukemia cells from 71 adult patients with acute leukemia (aged ≥ 12 years old) was detected by flow cytometry. The expression of CCR7 in adult patients with acute leukemia and its relationship with age, gender, diagnosis, peripheral blood WBC Number of bone marrow primitive naive cells ratio, CD56 expression, molecular biology, cytogenetics, risk stratification, extramedullary infiltration and other clinical features. Results: Of the 19 patients with acute lymphoblastic leukemia (ALL), 7 (36.8%) had CCR7 + and 5 (9.6%) had CCR7 + in 52 patients with acute myeloid leukemia (AML) The incidence of extramedullary infiltration in CCR7 + group was significantly higher than that in CCR7- group (100% vs 41.7%) (P <0.05), and ALL was associated with extramedullary infiltration group The average fluorescence intensity (MFI) of CCR7 was significantly higher than that of non-extramedullary infiltration (50.00 ± 10.42 vs 18.14 ± 1.39) (P <0.05). Conclusion: The expression of CCR7 is increased in adult acute leukemia. The positive rate of CCR7 in ALL is higher than that in AML and correlates with extramedullary infiltration.
