目的:研究齿突Ⅱ型骨折后枕颈部三维运动的变化规律,为临床选择治疗方法提供生物力学依据。方法:采用11具新鲜尸体的枕颈部骨韧带标本(C0~C3),应用脊柱三维运动测量及分析系统,对标本施加1.53Nm的纯力偶矩,就齿突Ⅱ型骨折后的枕颈部三维运动范围进行测量。结果:①寰枕关节的三维运动在齿突骨折后虽有增加,但无显著性意义;②寰枢关节三维运动范围有显著性增加的表现在旋转运动、前屈运动及屈伸、侧屈主运动时的藕合旋转运动。结论:齿突骨折后引起枕颈部不稳的主要原因系寰枢关节的轴向旋转不稳和前移不稳;手术治疗应以恢复齿突的完整和寰枢关节的稳定性为重点
OBJECTIVE: To study the changes of occipital and cervical three-dimensional motion of type II fractures, and to provide a biomechanical basis for clinical choice of treatment. Methods: 11 cases of fresh cadaveric cervical osseous ligament specimens (C0 ~ C3), the application of three-dimensional spinal motion measurement and analysis system, the sample applied a purely mechanical moment of 1.53Nm, the type Ⅱ fracture of the occiput neck Department of three-dimensional range of motion measurement. Results: ① The three-dimensional motion of atlanto-occipital joint was not significantly changed after the odontoid fracture; ② The three-dimensional motion range of atlantoaxial joint showed significant increase in rotational movement, flexion and extension, Coupling rotation during exercise. Conclusions: The main cause of unstable neck and occipital instability caused by odontoid fractures is the unstable axial rotation and unsteady movement of the atlantoaxial joint. The surgical treatment should focus on restoring the integrity of the odontoid process and the stability of the atlantoaxial joint
