目的探讨骨髓增殖性疾病(myeloproliferative disorders,MPD)患者骨髓中原癌基因独立生长因子(growth factor independence-1,Gfi1)的表达及其与临床预后的关系。方法选择华中科技大学同济医学院附属同济医院确诊的74例MPD患者为研究对象,其中真性红细胞增多(polycythaemia vera,PV)23例、原发性血小板增多症(essential thrombocythaemia,ET)49例,特发性骨髓纤维化(idiopathic myelofibrosis,IMF)2例。另选22例捐献骨髓的健康志愿者作为对照组。所有参与者签署知情同意书。采集骨髓标本以检测骨髓单个核细胞的Gfi1的表达。一步法提取细胞总RNA,应用逆转录-聚合酶链反应技术扩增Gfi1引物,琼脂糖凝胶电泳结果,分析各组MPD患者Gfi1阳性表达与MPD临床预后的关系。结果 74例MPD患者中52例检出Gfi1阳性表达,分别为ET患者67.35%(33/49)阳性表达,PV患者78.26%(18/23)阳性表达,IMF患者50.00%(1/2)阳性表达。Gfi1水平增高影响患者的预后,Gfi1阳性表达者,预后较差。结论 Gfi1可作为判断MPD预后的重要指标。随着对Gfi1的认识的深入,该基因很可能成为MPD治疗的分子靶点。
Objective To investigate the expression of growth factor independence-1 (Gfi1) in bone marrow of patients with myeloproliferative disorders (MPD) and its relationship with clinical prognosis. Methods Totally 74 patients with MPD confirmed by Tongji Hospital affiliated to Tongji Medical College of Huazhong University of Science and Technology were studied. Among them, 23 cases were polycythaemia vera (PV), 49 cases were essential thrombocythaemia (ET) Two cases of idiopathic myelofibrosis (IMF). Another 22 healthy volunteers donated bone marrow as a control group. All participants signed informed consent form. Bone marrow samples were collected to detect Gfi1 expression in bone marrow mononuclear cells. Total RNA was extracted by one-step method. Gfi1 primer and agarose gel electrophoresis were amplified by RT-PCR. The relationship between Gfi1 expression and MPD clinical prognosis was analyzed. Results The positive expression of Gfi1 was detected in 52 out of 74 MPD patients, which were 67.35% (33/49) in ET patients, 78.26% (18/23) in PV patients and 50.00% (1/2) in IMF patients, respectively expression. Gfi1 levels affect the prognosis of patients with Gfi1 positive expression, the prognosis is poor. Conclusion Gfi1 can be used as an important index to judge the prognosis of MPD. With the deepening of understanding of Gfi1, the gene is likely to become a molecular target of MPD therapy.
