目的观察调强放射治疗鼻咽癌的临床疗效,评价其临床应用价值。方法由病理证实的30例鼻咽癌患者,采用7个或9个野等角度分布照射:即300°、260°、220°、140°、100°、60°、30°、0°,用拓能的调强放射治疗系统进行逆向计划设计,再经过正向计算给出最后结果。采用6mv-X射线,共26次,GTV1+GTV2+GTV3=70Gy;CTV总量50Gy。结果放射治疗结束后完全缓解CR25例,3个月后复查又有2例达CR,CR率为90%,部分缓解PR3例,有效率100%。1年生存率100%。结论调强放射治疗鼻咽癌通过准确的CT定位,三维治疗计划系统的精确计算使靶区剂量集中均匀,而靶区以外剂量锐减,从而达到对靶区的高剂量照射和对其周围重要结构及正常组织的保护作用,易于被患者接受、安全、疗效好,疗中、疗后无痛苦。既可应用于首治患者,也可用于放射治疗后残留或复发的患者。
Objective To observe the clinical effect of intensity modulated radiation therapy on nasopharyngeal carcinoma and evaluate its clinical value. Methods Thirty cases of nasopharyngeal carcinoma confirmed by pathology were irradiated with seven or nine equiangular angles: 300 °, 260 °, 220 °, 140 °, 100 °, 60 °, 30 °, 0 ° with Extension to the intensity modulated radiation therapy system for reverse planning design, and then through the forward calculation gives the final result. Conclusion Intensity modulated radiation therapy of nasopharyngeal carcinoma through accurate CT positioning, accurate calculation of three-dimensional treatment planning system so that the target dose concentration, and sharp drop dose outside the target area, so as to achieve the target area of high dose irradiation and around the important Structure and protection of normal tissue, easy to be accepted by patients, safety, efficacy, treatment, after treatment without pain. Can be used in the first treatment of patients can also be used after radiotherapy residual or recurrence of patients.
