患者女性,49岁,临床诊断冠心病。于1983年5月19日连续描记心电图Ⅱ导联(附图)。一、P波多数直立,顺序发生,P-P0.75秒,P-R0.18秒,窦性下传QRS呈qR型,时间0.08秒,R波降支近顶端处略有挫折,为窦性心律。二、房性QRS和窦性者相同,前与逆行P(P′)相关,P′-R0.16秒,为起源于心房下部节奏点;其和前一心搏距离P-P′为0.60—0.70秒,P′-P′间最大公分母平均值1.505±0.025秒,变异系数为±1.66%,可判为房性并行心律(APSR)。三、定性QRS呈“R”型,时间0.12秒,“R”升支明显错折,前无相关P波。b行R_1后呈“正负双向”P′波,R-P′0.20秒,为“R”型QRS逆传心房和窦P之房性融合波。“R”型QRS之间最大公分母平均值0.408±0.018秒,变异系数±4.41%,a行R_7前有逆P,R_7形态介于房性和室性之间,为双重异位灶之室性融合波。联律间距0.52—0.68秒,为定性并行心律(VPSR)。
Female patient, 49 years old, with clinical diagnosis of coronary heart disease. On May 19, 1983 continuous tracing ECG lead II (with photos). First, the P wave most of the upright, in order, P-P0.75 seconds, P-R0.18 seconds, sinus QRS was qR type, time 0.08 seconds, R wave descending branch near the tip of a slight setback for the sinus Sexual rhythm. B line R_1 showed “positive and negative bidirectional” P wave, R-P’0.20 seconds for the “R” QRS retrograde atrial and sinus P fusion of atrial fibrillation. The mean value of the greatest common denominator of QRS was 0.408 ± 0.018second, the coefficient of variation was ± 4.41%. The reverse P before R_7 was located between atrial and ventricular.
