目的探讨生育政策调整前后松江区孕产妇在孕期保健及妊娠结局方面的差异,为更好地保障母婴健康提供依据。方法收集2014—2016年在松江区妇幼保健院分娩的27 862例孕产妇的孕期情况,分析其妊娠结局。结果 3年间,低龄孕产妇从0.69%降至0.34%,高龄孕产妇从5.16%升至9.99%;因“前次剖宫产”行再次剖宫产的比例从29.42%升至43.52%;先兆子痫及子痫、妊娠期糖尿病、妊娠期贫血及产后出血的患病率都有所上升。结论随着生育政策的调整,累积生育需求集中释放,高龄孕产妇比例增加,妇幼健康的服务质量和服务资源面临新的挑战。
Objective To explore the differences in maternal health and pregnancy outcomes before and after the birth policy adjustment in order to provide evidence for better protection of maternal and infant health. Methods The pregnancy status of 27 862 pregnant women who were delivered at MCH in Songjiang District during 2014-2016 were collected and their pregnancy outcomes were analyzed. Results During the three years, the number of pregnant women of the younger age decreased from 0.69% to 0.34%, and that of the elderly pregnant women rose from 5.16% to 9.99%; the rate of cesarean section increased from 29.42% to 43.52% after the “previous cesarean section” ; Pre-eclampsia and eclampsia, gestational diabetes mellitus, gestational anemia and postpartum hemorrhage prevalence rates have increased. Conclusions With the adjustment of the fertility policy, the demand for cumulative fertility has been released in a concentrated manner and the proportion of elderly pregnant women and mothers has increased. The quality of service and service resources for women and children are facing new challenges.
