目的探讨在外周静脉中心静脉置管(PICC)术后发生的机械性静脉炎治疗中应用红外线照射联合穿刺点碘伏棉球压迫的疗效。方法选择2010年6月至2016年6月行PICC置管后发生静脉炎的乳腺癌女性患者60例,随机分为两组。观察组30例,应用红外线照射每天两次,每次30 min,同时进行穿刺点碘伏棉球压迫;对照组30例,应用常规湿热敷的方法。观察两组疗效。结果观察组18例Ⅰ级患者3 d治愈,6例Ⅱ级患者6 d治愈,6例Ⅲ级患者6 d治愈,无一例提前拔管。对照组20例Ⅰ级患者3 d治愈,5例Ⅱ级患者6 d治愈;5例Ⅲ级患者配合金黄散湿敷,6 d症状有改善,红、肿、热、痛均未完全消退,其中3例持续12 d后消退,2例红肿症状反复出现并伴有体温升高而提前拔管。结论红外线照射与穿刺点碘伏棉球压迫是一种有效治疗PICC静脉炎的方法,特别是对乳腺癌患者有特殊的意义。
Objective To investigate the curative effect of infra-red radiation combined with punctate point-injection of iodophor in the treatment of mechanical phlebitis after periprocedural central venous catheterization (PICC). Methods Sixty female breast cancer patients with phlebitis after PICC catheterization from June 2010 to June 2016 were randomly divided into two groups. The observation group of 30 cases, the application of infrared radiation twice a day, every 30 min, while the puncture point povidone-cotton balls; control group of 30 cases, the application of conventional hot and humid wrap method. The two groups were observed. Results 18 cases of grade Ⅰ in the observation group were cured on day 3, 6 cases of grade Ⅱ were cured on day 6, and 6 cases of grade Ⅲ were cured on 6 days. None of them were extubated in advance. In the control group, 20 cases of grade Ⅰ patients were cured on day 3 and 5 cases of grade Ⅱ patients were cured on day 6; 5 cases of grade Ⅲ patients were treated with golden yellow loose wet compress. The symptoms improved on day 6, and redness, swelling, heat and pain did not completely disappear 3 cases continued to dissipate after 12 days, 2 cases of recurrence of symptoms of swelling and accompanied by elevated body temperature and extubation. Conclusion Infrared radiation and puncture point povidone-iodine compression is an effective method of treating PICC phlebitis, especially for patients with breast cancer.
