目的探讨血小板和血钙水平变化对于患儿病情监测的价值,为患儿的临床治疗提供帮助。方法随机选取2013年6月至2015年7月清远市第二人民医院收治的儿科重症住院患儿60例为研究对象,作为观察组,根据APACHEⅡ评分系统在患儿入院24 h内对病情严重程度进行分级,选取同期住院与门诊留观患儿60例为对照组,比较两组患儿及不同病情危重程度患儿血小板和血钙水平变化,观察患儿血小板减少发生率和低血钙发生率。结果观察组患儿血小板计数、血钙水平显著低于对照组,差异有统计学意义(P<0.05);随着患儿病情加重,血小板计数与血钙水平下降越明显;极危重症组患儿血小板减少症发生率、低血钙发生率及病死率均显著高于危重症组,差异有统计学意义(P<0.05)。结论危重症患儿血小板与血钙水平与病情严重程度呈正相关,监测血小板和血钙水平变化可判断患儿的病情程度,防止病情加重。
Objective To investigate the value of platelet and serum calcium levels in the monitoring of children’s condition and to provide help for the clinical treatment of children. Methods Sixty children with pediatric intensive care admitted to the Second People’s Hospital of Qingyuan City from June 2013 to July 2015 were selected randomly as the observation group. According to the APACHEⅡscore system, the severity of the illness 60 cases of hospitalized and outpatient observing children in the same period were selected as the control group. The changes of platelet and serum calcium levels were compared between the two groups of children and the children with different severity of illness. The incidence of thrombocytopenia and the incidence of hypocalcemia in children were observed . Results The platelet count and the level of serum calcium in the observation group were significantly lower than those in the control group (P <0.05). With the aggravation of the disease in children, the platelet count and serum calcium level decreased more obviously. In the critically ill group The incidence of thrombocytopenia, hypocalcemia and mortality were significantly higher than those in critically ill patients, the difference was statistically significant (P <0.05). Conclusion The levels of platelet and serum calcium in critically ill children are positively correlated with the severity of the disease. Monitoring the changes of platelet and serum calcium can determine the severity of the illness and prevent the exacerbations.
