目的研究胆道疾病儿童肝组织中热休克蛋白72(HSP72)与肝脏纤维化关系及其表达的意义。方法收集2013年6月至2015年6月深圳市儿童医院小儿肝胆外科行手术的胆总管囊肿(A组)和胆道闭锁(B组)患儿各30例的临床和病理资料,采用SP免疫组化法测定肝脏组织中HSP72的表达。结果肝纤维化Ohkuma′s分级B组高于A组(Z=7.04,P<0.01);HSP72染色阳性率B组高于A组(χ~2=41.71,P<0.01);HSp72表达强度B组高于A组(Z=6.31,P<0.01);B组中HSP72在肝纤维化程度较重组的表达低于较轻组(Z=-4.16,P<0.01),B组中HSP72表达强度与手术时年龄呈负相关(r=-0.704,P<0.01),与术后胆管炎发作呈负相关(r=-0.371,P<0.05),B组中HSP72表达强度较高者其自体肝生存率较高(χ2=9.482,P<0.01)。结论胆道疾病患儿肝脏组织中HSP72的表达与肝组织纤维化程度密切相关,HSP72表达强度可能为胆道闭锁患儿预后的影响因素之一,可作为评价预后的参考指标。
Objective To study the relationship between the expression of heat shock protein 72 (HSP72) and hepatic fibrosis in liver of children with biliary tract diseases. Methods The clinical and pathological data of 30 cases of choledochal cyst (group A) and biliary atresia (group B) were collected from Children’s Hospital of Shenzhen Children’s Hospital from June 2013 to June 2015. SP immunohistochemistry Assay of HSP72 expression in liver tissue. Results The positive rate of HSP72 in group B was higher than that in group A (χ ~ 2 = 41.71, P <0.01), while the expression of HSp72 in group B was higher than that in group A (Z = 7.04, P <0.01) Group B was higher than group A (Z = 6.31, P <0.01). In group B, the expression of HSP72 was lower in severe group with liver fibrosis (Z = -4.16, P <0.01) (R = -0.704, P <0.01), and was negatively correlated with postoperative cholangitis (r = -0.371, P <0.05). The expression of HSP72 in group B was higher than that in autologous liver The survival rate was higher (χ2 = 9.482, P <0.01). Conclusion The expression of HSP72 in liver tissue of children with biliary tract diseases is closely related to the degree of liver fibrosis. The expression intensity of HSP72 may be one of the influencing factors in the prognosis of children with biliary atresia, which may be used as a reference index to evaluate the prognosis.
