早产儿早期及日间血浆β-内啡肽变化趋势研究

目的 探讨早产儿血浆β-内啡肽(β-EP)在生后7 d及日间的变化规律。方法 选择母无产科合并症的早产儿20例,分别于出生后0 h、12 h、24 h、3 d、5d、7 d用放射免疫法测定其血浆β-EP水平。并对生后3~5d的早产儿分9AM、12PM、3PM三个时间点采血,测定其血浆β-EP水平。结果 早产儿在出生后0 h、12h、24 h、3 d、5 d、7 d的血浆浓度(pg/ml)分别为439.2±217.7、677.3±228.4、426.4±215.8、446.9±206.5、426.5±194.9、328.7±169.1。以出生后12 h血浆β-EP浓度最高,与其它组比较差异无显著性(P均>0.05)。生后24 h至生后7 d血浆β-EP水平变化不大,各组间比较差异无显著性(P>0.05)。生后3-5 d的早产儿在9AM、12PM、3PM的血浆β-EP水平(pg/ml)分别为464.6±224.4、413.7±212.0、390.2±201.9,有逐渐下降趋势,但各组间比较差异无显著性(P>0.05)。结论早产儿在出生后12 hβ-EP水平最高,生后24h降为出生时水平,以后6 d变化不大,这种生后12h高水平血浆β-EP可能对调节新生儿疼痛起着重要作用。血浆β-EP浓度在白天有逐渐下降趋势。 Objective To investigate the changes of plasma beta-endorphin (β-EP) in preterm infants on day 7 and day after birth. Methods Twenty maternal preterm infants with obstetric complications were selected and their plasma levels of β-EP were measured by radioimmunoassay at 0 h, 12 h, 24 h, 3 d, 5 d and 7 d after birth. Blood samples were collected at 3 AM, 9 AM, 12 PM and 3 PM after 3 ~ 5 days of postnatal day. The plasma levels of β-EP were measured. Results The plasma concentrations (pg / ml) of preterm infants at 0 h, 12 h, 24 h, 3 d, 5 d, and 7 d after birth were 439.2 ± 217.7, 677.3 ± 228.4, 426.4 ± 215.8, 446.9 ± 206.5, 426.5 ± 194.9, 328.7 ± 169.1. The plasma β-EP was the highest at 12 h after birth, and no significant difference compared with other groups (all P> 0.05). Plasma β-EP level did not change much between 24 h and 7 d after birth, and there was no significant difference among the groups (P> 0.05). Plasma β-EP levels (pg / ml) were 464.6 ± 224.4, 413.7 ± 212.0 and 390.2 ± 201.9 in preterm infants 3-5 days after birth at 9 AM, 12 PM and 3 PM, respectively, which showed a gradual downward trend. However, There was no significant difference (P> 0.05). Conclusions The level of β-EP in the preterm infants at 12 h after birth is the highest, at 12 h after birth, it drops to the level at birth, and does not change much after 6 d. This high plasma β-EP 12 h after birth may play an important role in the regulation of neonatal pain . Plasma β-EP concentration gradually declines during the day.
