目的:探讨参麦注射液对肺炎合并心力衰竭患儿的疗效及心功能的保护作用。方法:选取2011~2013年该院收治的84例重症肺炎合并心力衰竭患儿为研究对象,根据随机数字表将患儿分为观察组及对照组各42例,对照组患儿给予常规的利尿、强心、抗感染治疗,观察组在对照组基础上应用参麦注射液治疗。治疗前后采用彩色多普勒血流仪测定两组患儿舒张期室间隔厚度(IVST)、左心室射血分数(LVEF)、左室舒张末内径(LVDD)水平。采用日立全自动生化分析仪测定两组患儿心肌酶谱(CK、AST、CK-MB、LDH、HBDH)。结果:观察组患儿咳嗽消失时间、心衰纠正时间、肺啰音消失时间、气急消失时间、平均住院时间均优于对照组,差异有统计学意义(P<0.05)。观察组治疗后IVST、LVEF、LVDD改善效果优于对照组,差异有统计学意义(P<0.05)。观察组治疗后心肌酶谱(CK、AST、CK-MB、LDH、HBDH)各指标显著低于对照组,差异有统计学意义(P<0.05)。结论:参麦注射液能有效改善肺炎合并心力衰竭患儿的临床症状,保护患儿心功能,减轻对心肌的损害,促进患儿预后。
Objective: To investigate the effect of Shenmai injection on the curative effect and cardiac function in children with pneumonia complicated with heart failure. Methods: Eighty-four children with severe pneumonia complicated with heart failure admitted in our hospital from 2011 to 2013 were selected as study objects. According to the random number table, 42 children were divided into observation group and control group, while those in control group were given routine diuresis , Cardiac, anti-infective treatment, the observation group in the control group based on the application of shenmai injection. Before and after treatment, the left ventricular ejection fraction (LVEF) and left ventricular end diastolic diameter (LVDD) were measured by color Doppler flowmeter in two groups. The myocardial enzymes (CK, AST, CK-MB, LDH, HBDH) of two groups were measured by Hitachi automatic biochemistry analyzer. Results: The disappearance time of cough, heart failure correction time, disappearance of pulmonary rales, disappearance of shortness of breath and average length of stay in observation group were better than those in control group (P <0.05). The improvement effect of IVST, LVEF and LVDD in observation group was better than that in control group, the difference was statistically significant (P <0.05). The indexes of myocardial enzymes (CK, AST, CK-MB, LDH and HBDH) in the observation group were significantly lower than those in the control group after treatment (P <0.05). Conclusion: Shenmai injection can effectively improve the clinical symptoms of children with pneumonia complicated with heart failure, protect the cardiac function, reduce the damage to the myocardium and promote the prognosis of children.
