目的:探讨直接涂片ABC蛳AP染色在急性白血病免疫分型诊断中的价值。方法:对102例急性白血病患者的骨髓或外周血直接涂片,应用13种白细胞系列单克隆抗体,ABC免疫酶标法进行免疫学分型。结果:在73例ANLL中,纯髓系表型占90.4 %(66/73),粒单系特异性标记单抗表达率高低依次为CD+13 80.6%(58/72);CD+33 68.1 %(49/72);CD+15 55.6 %(40/72);29例ALL中,纯淋巴系表型为93.1%(27/29)。B细胞系特异性单抗标记表达率高低依次为CD+19 83.3 %(20/24);CD+22 70.8 %(17/24);CD+10 41.7 %(10/24)。T细胞系标记表达率高低依次为CD+5 和CD+7均为100 %(5/5);CD+2 80 %(4/5);CD+3 60 %(3/5)。102例急性白血病中,形态学分型正确归类者为96例,符合率为94.1 %(96/102)。结论:直接涂片ABC蛳AP染色,方法简便易行,结果可靠。免疫分型对急淋与急非淋鉴别诊断及对少见类型急性白血病的诊断具有重要的临床价值
Objective: To investigate the value of direct smear ABC 蛳 AP staining in the diagnosis of acute leukemia immunophenotype. Methods: 102 cases of acute leukemia patients with bone marrow or peripheral blood smear, the application of 13 kinds of white blood cell monoclonal antibody, ABC immunoenzyme method for immunological typing. RESULTS: The phenotypes of pure myeloid lineages in 90 ANLL patients were 90.4% (66/73). The expression rates of single line specific monoclonal antibodies were in order of CD + 13 80.6% (58/72) and CD + 33 68.1 % (49/72); CD + 15 55.6% (40/72). The phenotype of pure lymphatic system was 93.1% (27/29) in 29 cases of ALL. The positive rate of B cell line specific McAb was CD + 19 83.3% (20/24); CD + 22 70.8% (17/24); CD + 10 41.7% (10/24). The expression of T-cell marker was in the order of 100% (5/5) for CD + 5 and CD + 7; 80% (4/5) for CD + 2 and 60% (3/5) for CD + 3. In 102 cases of acute leukemia, 96 cases were correctly classified by morphological classification, the coincidence rate was 94.1% (96/102). Conclusion: Direct smear ABC 蛳 AP staining, the method is simple and easy, the results are reliable. Immunophenotyping has an important clinical value for the differential diagnosis of acute lymphocytic and non-lymphocytic leukemia and the diagnosis of rare type of acute leukemia
