目的:探讨自主神经对心电图波形产生的影响。方法:对照分析胆道疾病患者阿托品试验心电图P波与QRS波的改变,分析自主神经的作用及其临床意义。结果:阿托品试验心电图的主要变化:①全部P波振幅增高、时限延长(均P>0.05),△Pw(3.7±3.6)ms、△Ph(1.4±1.2)mm。②大部分Ⅱ导联(85.4%)和V5导联(88.5%)R波振幅降低(均P>0.05),△Rh分别为(0.83±0.79)mm和(1.78±1.28)mm。③P波振幅增幅老年人大于中青年人(P>0.05),时限增幅青年人大于老年人(P<0.01);心率100~120次/min时△Pw、△Ph增幅最大(P<0.01和P>0.05);V5△Rh降幅最大(P<0.01)。结论:阿托品试验的P波与QRS波改变主要表现为,P波振幅增高、时限延长,R波振幅降低。与自主神经张力及年龄、心率有关。
Objective: To investigate the effect of autonomic nerve on the electrocardiogram waveform. Methods: The changes of P wave and QRS wave of atropine in patients with biliary tract disease were analyzed. The function of autonomic nerve and its clinical significance were analyzed. Results: The main changes of electrocardiogram in atropine test were as follows: ①The amplitude of all P wave was increased and the time limit was prolonged (all P> 0.05), △ Pw (3.7 ± 3.6) ms and △ Ph (1.4 ± 1.2) mm. ② The amplitudes of R wave in most lead II (85.4%) and V5 lead (88.5%) were decreased (all P> 0.05), △ Rh were (0.83 ± 0.79) mm and (1.78 ± 1.28) mm, respectively. (3) The amplitudes of amplitude of P wave in the elderly were higher than those in the middle-aged and young people (P <0.05), and the increase of the time limit was higher in the young people than in the elderly (P <0.01). The △ Pw and △ Ph increased the most when the heart rate was 100-120 beats / > 0.05); V5 △ Rh decline the largest (P <0.01). Conclusion: The changes of P wave and QRS wave in atropine test are mainly as follows: the amplitude of P wave increases, the time limit prolongs, and the amplitude of R wave decreases. And autonomic nervous tension and age, heart rate.
