目的:分析CT扫描、食管造影和内窥镜测量食管癌病变长度的差异及其一致程度.方法:应用CT扫描、食管造影和内窥镜等不同检查手段测量74例食管癌病变长度,以食管癌病理标本根据收缩比回推至人体内实际长度为标准进行比较,分析各检查方法测量食管癌病变长度的差异并进行一致性分析,探讨各诊断方法在判断食管癌病变长度方面的价值.结果:①食管癌病变长度从小到大依次为镜检长度、实体肿瘤长度、食管造影长度和CT扫描长度,其中镜检长度和造影长度与实体肿瘤长度较为接近.②CT扫描与食管造影对病变长度判断存在一致性(P=0.032),但一致性差(Kappa=0.152);CT扫描与内窥镜、食管造影与内窥镜对长度判断结果均不一致(P:0.914和P:0.456).结论:三种检查方法测量食管癌病变长度存在一定差异,食管癌合理的病变长度要结合钡餐造影、内镜和cT等检查手段来综合判断.
