目的比较全胃切除术患者经留置鼻空肠营养管与空肠造口行肠内营养的临床效果。方法将淮安市第一人民医院2006年7月至2010年12月进行全胃切除术治疗的57例患者按照肠内营养方法不同分为两组,留置鼻空肠营养管的30例作为对照组,空肠造口的27例作为观察组,比较分析两组的临床效果。结果观察组的肛门排气时间、排便时间及住院时间均明显短于对照组,且观察组术后第8d各项营养指标均明显优于对照组,组间差异均有统计学意义(P<0.05);观察组的并发症发生率为7.4%,略低于对照组的13.3%,但组间差异无统计学意义(P>0.05)。结论全胃切除术患者经空肠造口行肠内营养临床效果更为满意,能明显改善患者的营养状态,并且使患者胃肠功能恢复更快,有利于患者的康复,是较佳的肠内营养方法之一。
Objective To compare the clinical effects of enteral nutrition in patients with total gastrectomy by nasal jejunal feeding tube and jejunum stoma. Methods Fifty-seven patients underwent total gastrectomy in Huai’an First People’s Hospital from July 2006 to December 2010 were divided into two groups according to different enteral nutrition methods. Thirty patients with nasal jejunal feeding tube were used as control group, 27 cases of jejunostomy as observation group, comparative analysis of the clinical effect of the two groups. Results The anal exhaust time, defecation time and hospital stay in the observation group were significantly shorter than those in the control group, and the nutritional indexes of the observation group were significantly better than those of the control group on the 8th day after operation. The differences between the two groups were statistically significant (P < 0.05). The complication rate in the observation group was 7.4%, slightly lower than that in the control group (13.3%), but there was no significant difference between the two groups (P> 0.05). Conclusions Total gastrectomy patients with jejunostomy enteral nutrition clinical effect is more satisfied, can significantly improve the nutritional status of patients, and patients with gastrointestinal function recovery faster, is conducive to the recovery of patients, is the preferred enteral One of the nutritional methods.
