目的探讨烯丙雌醇片治疗黄体功能不全所致先兆流产的临床效果。方法选取2015年6月至2017年3月龙川县妇幼保健院收治的先兆流产患者80例为研究对象,按随机数字表法分为对照组和研究组,每组40例。所有患者入院后均严格执行医嘱,绝对卧床休息并绝对禁止性生活,口服叶酸,阴道流血>3 d者给予抗感染药物预防和治疗感染。对照组患者给予黄体酮注射液,研究组患者给予烯丙雌醇片,对所有患者均定期门诊随访,病情严重者住院治疗,比较两组患者孕6周、8周、10周体内孕酮水平变化情况及干预后两组血清雌二醇(E_2)和孕激素(PRL)水平,并观察两组患者整体保胎情况。结果研究组患者孕6周、8周、10周体内孕酮水平均显著高于对照组,差异有统计学意义(P<0.05);干预后,研究组患者E_2及PRL基本处于正常水平,且均显著优于对照组,差异有统计学意义(P<0.05);研究组患者保胎成功率显著高于对照组,差异有统计学意义(P<0.05)。结论烯丙雌醇治疗黄体功能不全所致先兆流产,能有效调节患者体内激素水平,提高保胎成功率。
Objective To investigate the clinical effect of allyl estradiol tablets in the treatment of threatened abortion caused by luteal insufficiency. Methods Eighty patients with threatened abortion admitted to Longchuan MCH Hospital from June 2015 to March 2017 were selected as the research object and randomly divided into control group and study group with 40 cases in each group. All patients were strictly enforced after admission, doctor’s advice, absolute bed rest and absolute prohibition of sex life, oral folic acid, vaginal bleeding> 3 d were given anti-infectives to prevent and treat infections. Patients in the control group were given progesterone injection. Patients in the study group were given allyl estradiol tablets. All patients were regularly outpatient follow-up, hospitalized in serious condition. The levels of progesterone in the two groups were compared at 6 weeks, 8 weeks and 10 weeks Changes and intervention of serum estradiol (E2) and progesterone (PRL) levels in both groups, and the two groups of patients overall miscarriage. Results The level of progesterone in study group was significantly higher than that in control group at 6 weeks, 8 weeks and 10 weeks (P <0.05). After intervention, E 2 and PRL in study group were basically normal, and (P <0.05). The successful rate of miscarriage in study group was significantly higher than that in control group, the difference was statistically significant (P <0.05). Conclusion The treatment of threatened abortion induced by albuterol in patients with luteal insufficiency can effectively regulate hormone levels in patients and improve the success rate of miscarriage.
